Is DM a painful disease? No. Degenerative Myelopathy is not a painful condition, but it does weaken pets. Dogs with DM will struggle, which may be painful to watch, but they aren't experiencing any pain.
As horrible as all of this sounds, degenerative myelopathy is not painful That being said, dogs that suffer from degenerative myelopathy may become sore from overuse of other areas of their body while trying to compensate for their hind end weakness.
DM is an inherited neurologic disease typically affecting dogs around eight years of age or older. It causes gradual muscle wasting and incoordination in the hind limbs, and then progresses to an inability to walk after six to twelve months.
Sadly, most dogs with degenerative myelopathy eventually lose control of their legs, bladder and bowels completely, and need to be put to sleep. The average life expectancy of a dog with degenerative myelopathy is 1-2 years from diagnosis.
How Quickly Does Degenerative Myelopathy Progress? Unfortunately, DM tends to progress very quickly. Most dogs that have been diagnosed with degenerative myelopathy will become paraplegic within six months to a year.
Late Stage
The clinical signs of late-stage disease include: Complete paralysis of the hind limbs and loss of all motor function. Loss of all sensory and deep-pain reflexes. Hyporeflexia of the myotatic and withdrawal reflexes.
STAGE 4 – LMN tetraplegia and brain stem signs (~ over 36 months) – At the end of the disease, the degeneration can progress to involve neck, brain stem, and brain. Patients will not be able to move all four limbs, have trouble breathing, and have difficulty with swallowing and tongue movement.
With time, the disease causes paralysis of the hind limbs. The entire spinal cord can eventually become affected—causing forelimb paralysis and problems with breathing, vocalizing, and eating.
What are the typical signs of pain in dogs? General behaviour: Shaking, flattened ears, low posture, aggression, grumpy temperament, panting or crying, excessive licking or scratching a specific area, reluctant to play, interact or exercise, lameness (limping), stiffness after rest, loss of appetite.
There is no effective treatment for degenerative myelopathy at present. Treatment of other concurrent problems such as arthritis or hip dysplasia may provide some relief from pain or discomfort. It is important to avoid obesity, so diet and exercise (walking and swimming) are vital components of treatment.
In its final stages, degenerative myelopathy can spread to the brain stem. This impacts cognitive function as well as the ability to breathe and swallow normally.
Canine Degenerative Myelopathy (DM) is a progressive disease of the spinal cord and ultimately the brain stem and cranial nerves which, at its end stages, results in complete paralysis and death.
Left untreated, myelopathy can get worse over time. Spinal compression can permanently damage the nerves that control essential body movements and function. You can't reverse nerve damage, so it is important to get care as soon as you notice symptoms.

Stiffness and limping are two of the more obvious signs of pain in dogs and are likely a result of injury, sore paws, or even arthritis. Your dog might be reluctant to climb stairs or is noticeably slow when getting up. This can also manifest itself as reduced interest in exercise, or not being as active as usual.
Research at the University of Lincoln, UK, found that dogs, “can recognize emotions in humans by combining information from different senses.”[1] Your dog recognizes when you're sad or in pain by facial recognition and body language, but the most interesting sense they use is smell.
Dr. Gladstein says she often tells pet parents to place a heating pad on low power on the dog's back for 10 minutes every hour to help relieve pain. That, combined with a few weeks of rest, is sometimes all that's needed for the dog to heal from a mild back injury.
Differential Diagnosis
14 Disorders that often mimic and coexist with DM include degenerative lumbosacral syndrome, intervertebral disc disease, spinal cord neoplasia and degenerative joint diseases such as hip dysplasia or cranial cruciate ligament rupture.
Canine degenerative myelopathy (DM) was previously known as chronic degenerative radiculomyelopathy (CDRM) and is a progressive degenerative spinal cord disease. Ultimately it is a fatal disease with devastating consequences for the dogs and can be distressing for owners caring for them.
The definitive therapy for DM in dogs is insulin, to replace the deficiency caused by lack of functional pancreatic beta cells. A short-acting insulin such as regular insulin has a rapid onset of action; is degraded quickly; and may be given by the intravenous, intramuscular, or subcutaneous routes.

Myelopathy describes any neurologic symptoms related to the spinal cord and is a serious condition. It occurs from spinal stenosis that causes pressure on the spinal cord. If untreated, this can lead to significant and permanent nerve damage including paralysis and death.
How long do dogs live with Degenerative Myelopathy? Dogs generally live with DM for anywhere between six months and three years. Rehab therapy and regular exercise can help improve the quality of life for dogs with DM. Unfortunately, Degenerative Myelopathy has no cure at the moment.
