In the case of meniscus tears, some people think the injury will heal over time on its own. But the truth is that there are different types of meniscus tears — and some tears won't heal without treatment. If your tear is on the outer one-third of the meniscus, it may heal on its own or be repaired surgically.
In arthroscopy, part of the meniscus is removed. As a result, the knee has considerably less support and absorbancy. In time, osteoarthritis in the knee worsens, and the only option for successful treatment is a total knee replacement.
Meniscus tears are the most frequently treated knee injuries. Recovery will take about 6 to 8 weeks if your meniscus tear is treated conservatively, without surgery. If your symptoms persist after 3 months or your symptoms become significant, your doctor may recommend surgery to repair the tear.
Surgery to repair tears in the meniscus relieves symptoms 85% of the time. That means that of 100 people who have this surgery, 85 have relief from pain and can use their knee normally, while 15 do not. Surgery to repair tears may reduce the risk of long-term joint problems.
Continuing to walk on the affected leg can aggravate your symptoms, making pain and stiffness worse in as little as a few days. Plus, suffering a torn meniscus may also increase your risk of complications, like developing osteoarthritis in that knee.
Left untreated, a meniscus tear can limit your daily life and ability to participate in exercise and sports. In serious cases, it can develop into long-term knee problems, like arthritis.
“You can live with a meniscus tear,” Dr. Parker says. “And it may heal on its own, or just not cause you problems.” However, you'll know pretty quickly when it is a problem.
Symptoms may go away but can come back from overuse or when you do activities that involve twisting. The pain may come and go over a period of years if the tear isn't treated. Larger tears usually cause more pain and immediate swelling and stiffness.
Avoid activities that aggravate your knee pain, especially any activity that causes you to twist, rotate or pivot your knee. If your pain is severe, using crutches can take pressure off your knee and promote healing. Ice. Ice can reduce knee pain and swelling.
Some exercises are too strenuous for people with meniscus tears. A person should not: do deep squats. do any exercise that involves pivoting or that otherwise twists the knee.
To avoid placing stress on the meniscus repair, do not bend your knee beyond 90 degrees (bent to a right angle). Brace and Crutches: Unless otherwise instructed by your doctor, use crutches when walking and bear weight as tolerated on the operated leg. Wear the post-operative brace for walking.
What type of meniscus tear is the worst? Bucket-handle meniscus tears are the most serious type of meniscus injury. They often occur alongside a ligament injury. Bucket-handle tears require prompt surgery in order to have the best possible chance of recovery.
Tears on the outer one-third of your meniscus can heal independently because this area has a rich blood supply, and blood cells can regenerate the meniscus tissue. However, if the tear is located on the inner two-thirds, the tear will not repair itself and will need to be surgically removed.
Most people can walk with crutches soon after meniscus surgery. Many return to normal activities within six to eight weeks. Your healthcare provider may recommend low-impact activities rather than high-impact (such as walking rather than running). If you have a physical job, you may need extra time off work to recover.
What is the prognosis (outlook) for people with a torn meniscus? Most people who tear a meniscus can return to full activity. If you have surgery to repair a torn meniscus, your knee should be fully recovered after a few months of physical therapy.
You will have a physical examination to find out if you have a torn meniscus and to rule out other knee injuries. Your doctor will check both knees for tenderness, range of motion, and knee stability. X-rays are usually done. Based on your symptoms and the physical examination, your doctor may diagnose a meniscus tear.
Symptoms of a meniscus tear may be different for each person, but some of the most common symptoms are: Pain in the knee joint: usually on the inside (medial), outside (lateral) or back of the knee. Swelling. Catching or locking of the knee joint.
Swelling or stiffness. Pain, especially when twisting or rotating your knee. Difficulty straightening your knee fully. Feeling as though your knee is locked in place when you try to move it.
Mobility. A torn meniscus can prevent normal pain-free motion of the knee and therefore can interfere with the patient's ability to climb stairs or get in and out of chairs and cars. Sometimes the pain in the knee causes the brain to shut off the quadriceps muscle at the knee and leads to a "giving way" sensation.
Avoid positions and activities that place excessive pressure on knee until pain and swelling resolve. Problematic positions include squatting, pivoting, repetitive bending (eg, climbing stairs, rising from seated position, operating clutch and pedals), jogging, and swimming using the frog or whip kick.
Sleep on your back with the leg slightly elevated
This is especially important during the first few days after meniscus surgery. Keeping the leg elevated (but not bent!) encourages healthy circulation, which helps keep pain and swelling in check.
