Those with an anxiety disorder have most likely experienced a panic or anxiety attack at some point in their lives. The symptoms can closely mimic heart attacks for some people—they may feel chest pain, shortness of breath and heart palpitations, or a racing heartbeat.
The difference is that, when extra heartbeats in the upper and lower chambers are the cause of abnormal rhythm, symptoms may feel like an initial skip or hard thumping beat followed by a racing heart. When anxiety is the trigger, heart rate typically increases steadily rather than suddenly.
Panic disorder – can be associated with cardiac disease or mistaken for heart attack. Feelings of extreme agitation and terror are often accompanied by dizziness, chest pains, stomach discomfort, shortness of breath, and rapid heart rate.
Heart Palpitations and Anxiety. Heart palpitations due to anxiety feel like your heart is racing, fluttering, pounding or skipping a beat. Your heartbeat can increase in response to specific stressful situations. You may also have palpitations due to an anxiety disorder (excessive or persistent worry).
Although chest pain is common to both a panic attack and a heart attack, the characteristics of the pain often differ. During a panic attack, chest pain is usually sharp or stabbing and localized in the middle of the chest. Chest pain from a heart attack may resemble pressure or a squeezing sensation.
Signs of a heart attack include:
- Pain or discomfort in one or both arms, the back, neck, jaw or stomach. - Shortness of breath with or without chest discomfort. - Other signs such as breaking out in a cold sweat, nausea or lightheadedness. (If you're experiencing any of these symptoms, call 9-1-1 immediately.)
A heart attack may strike suddenly, but most people have warning signs and symptoms hours, days or weeks beforehand. One of the earliest warning signs of an impending heart attack is chest pain, or angina, that occurs repeatedly because of exertion and is then eased by rest.
How anxiety causes chest pain. When you're anxious, your brain sends a surge of adrenaline and cortisol through your body. These hormones immediately trigger a rapid rise in your heart rate and blood pressure. As a result, many people experience chest pain and sweating, or have a hard time breathing.
While anxiety chest pain varies from person to person, you may feel a constant, mild sense of discomfort or chest pain that comes and goes throughout the day. Panic attacks can last anywhere from five to 30 minutes.
How Long Does Anxiety Chest Pain Last? Though it can feel alarming, anxiety chest pain is fleeting. Pain typically lasts around 10 minutes, though other anxiety or panic attack symptoms (like dizziness, shortness of breath, or nausea) may last longer.

Sudden stress can cause a cardiac event that feels like a heart attack, called takotsubo cardiomyopathy or “broken heart syndrome.” This stress-induced cardiomyopathy isn't associated with the artery blockages that lead to a heart attack, though it may cause your heart to pump inefficiently for up to a month.

Anxiety could cause chest pain on the left side. However, it can also be a sign of a heart attack or pericarditis, so a person should seek immediate medical attention.
Stress, anxiety and depression can also manifest as chronic chest pain. Other conditions can cause short-term, acute chest pain, including lung problems and musculoskeletal injuries.

Can you have a heart attack and not know it? Yes. A heart attack can actually happen without a person knowing it. You can understand why it is called a "silent" heart attack.
You may not even know you've had a silent heart attack until weeks or months after it happens. It's best to know what's normal for your body and get help when something doesn't feel right.
Electrocardiogram. An electrocardiogram (ECG) is an important test in suspected heart attacks. It should be done within 10 minutes of being admitted to hospital. An ECG measures the electrical activity of your heart.
Catch the signs early
Pay attention to your body and call 911 if you experience: Chest discomfort. Most heart attacks involve discomfort in the center of the chest that lasts more than a few minutes – or it may go away and then return. It can feel like uncomfortable pressure, squeezing, fullness or pain.
A silent heart attack may resolve on its own if, for example, the clot blocking blood flow dissolves or becomes dislodged and is absorbed into the body. But a silent heart attack can still cause damage.
Anginophobia makes you afraid of chest pain (angina) or chest tightness. You may worry about problems, like choking, that cut off oxygen, causing chest pain and a potential heart attack. Some people also have illness anxiety disorder. They associate normal body functions with a serious medical problem.
Heartburn, angina and heart attack may feel very much alike. Even experienced doctors can't always tell the difference from your medical history and a physical exam. That's why, if you go to the emergency room because of chest pain, you'll immediately have tests to rule out a heart attack.
