Botox injections into the forehead muscle and the area between the eyes can cause a droopy or irritated eyelid. This adverse reaction is more likely if the person administering the treatment hasn't had enough training and experience. They may inject Botox in the wrong place or use a high dose.
Brow ptosis happens when Botox is injected too low into the frontalis muscle, the muscle that raises the eyebrows. It could also happen when the dose used is too high or if the Botox has been improperly diluted with normal saline or adrenaline.
Too much Botox in the forehead muscles can cause the eyebrows to droop, making the upper eyelids look very heavy and hooded. The face may look angry or sad all the time. Too much Botox around the eyes can dramatically affect facial expression. The face is simply frozen.

Signs of a Bad Botox Injection
Unusual results such as facial asymmetry (one side doesn't match the other), overarching eyebrows, a drooping eyelid, or not being able to move the forehead at all.
Botox can spread inferiorly and cause eyelid drooping if this area is injected more deeply. Injections placed too deeply in the frontalis muscle can spread botulinum toxin down the forehead, where it relaxes the lower muscle fibers and creates heaviness of the brow. A shallower depth is best here as well.
The glabella, or “brow bone”, is located between your eyebrows and eyes. It's known as a very sensitive area in the face—so if you inject it with botox (botulinum toxin), it can cause unwanted side effects like droopy eyelids and headaches.
Heaviness in your eyebrows after Botox can be fixed in a couple of ways. This will relax the muscles that pull down on your eyebrows and make them look and feel less heavy.

Botox Frequency
Not every patient's experience is the same, but the average time frame for Botox efficacy is around four months. After that time period, you'll start to see your original facial lines begin to re-form.
If a patient has a heavy upper eyelid and too much Botox is injected into the forehead, the eyes can look sunken or tired. Also, if more Botox is injected into the middle of the forehead than on the sides, the result can be what some people call a “Spock brow.”
Many unskilled injectors make the mistake of injecting Botox too deeply or too superficially into the skin. Improper placement. A skilled provider has excellent knowledge of intricate facial anatomy and therefore understands precisely where to place each unit of Botox to achieve the desired result.
