OCD was one of the first psychiatric disorders in brain scans showed evidence of abnormal brain activity in specific regions.
Brain scans may be helpful in showing the differences in the structure and function of brain regions in individuals with OCD. Such studies can provide new targets for the treatment of OCD.
Imaging, surgical, and lesion studies suggest that the prefrontal cortex (orbitofrontal and anterior cingulate cortexes), basal ganglia, and thalamus are involved in the pathogenesis of obsessive-compulsive disorder (OCD).
Diagnosis and Tests
There's no test for OCD. A healthcare provider makes the diagnosis after asking you about your symptoms and medical and mental health history. Providers use criteria explained in the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition (DSM-V) to diagnose OCD.
Once thought to be psychodynamic in origin, OCD is now generally recognized as having a neurobiological cause. Although the exact pathophysiology of OCD in its pure form remains unknown, there are numerous reports of obsessive-compulsive symptoms arising in the setting of known neurological disease.
OCD has also been associated with decreased cortical thickness in various frontal, parietal, and temporal cortical regions. These results highlight the existence of structural brain abnormalities outside of the classic orbitofrontal loop circuit.
Researchers know that OCD is triggered by communication problems between the brain's deeper structures and the front part of the brain. These parts of the brain primarily use serotonin to communicate. This is why increasing the levels of serotonin in the brain can help to alleviate OCD symptoms.
Experts aren't sure of the exact cause of OCD. Genetics, brain abnormalities, and the environment are thought to play a role. It often starts in the teens or early adulthood. But, it can also start in childhood.
Obsessive compulsive disorder, or OCD, is an anxiety disorder which, like many anxiety disorders, is marked by low levels of serotonin. Serotonin, a type of neurotransmitter, has a variety of functions that make a deficiency a serious and anxiety producing issue.
Brain structure and function
Studies show that OCD patients have excess activity in frontal regions of the brain, including the orbitofrontal cortex (OFC) and anterior cingulate cortex (ACC), which could explain their intrusive thoughts and high levels of anxiety, respectively.
How is OCD diagnosed? There is no single test for OCD, but a health professional can make a diagnosis based on an assessment of the person's behaviours, thoughts and feelings. To be diagnosed as having OCD, obsessive thoughts and compulsive behaviours must be: taking up a lot of time (more than 1 hour a day)
Some primary care physicians do diagnose OCD correctly. But in general, when people with OCD are able to get evaluated by a specialist experienced in OCD treatment, they're much more likely to receive an accurate diagnosis and empirically supported treatment– usually a combination of medications and ERP therapy.
A standard treatment for OCD involves exposure and response prevention (ERP). This involves the patient confronting their worst fears while learning to not perform their compulsions. For example, it may include touching a toilet seat and not being allowed to wash your hands.

Reduced dopamine D1 receptors and dopamine D2 receptors in the striatum have been reported in people with OCD, along with both increased and decreased reports of dopamine transporter (DAT) binding.
Is OCD Caused by a Chemical Imbalance? Changes in the neurochemical serotonin, as well as in the neurochemicals dopamine and glutamate, are likely present in OCD. Indeed, medications like the antidepressants known as selective serotonin reuptake inhibitors (SSRIs) improve symptoms for many people.
Compulsive behaviors related to OCD likely derive from a similar imbalance in the dopaminergic reward system. Perhaps continuous activation of the ventral striatum creates an undirected expectation for reward. The unfulfilled expectation causes an anxious feeling and an urge to satisfy the void.
Is OCD Inherited? Research shows that OCD does run in families, and that genes likely play a role in the development of the disorder. Genes appear to be only partly responsible for causing the disorder, though.
Results: Emotional abuse, sexual abuse and neglect were highly prevalent in our sample. Additionally, the severity of experienced childhood maltreatment was associated with higher OCD symptom severity, with the strongest association found for emotional abuse.
Getting recovered takes time
Speaking from experience, I would say that the average uncomplicated case of OCD takes from about six to twelve months to be successfully completed. If symptoms are severe, if the person works at a slow pace, or if other problems are also present, it can take longer.
