Compulsions are repetitive activities that you do to reduce the anxiety caused by the obsession. It could be something like repeatedly checking a door is locked, repeating a specific phrase in your head or checking how your body feels.
What is speaking related OCD? People with Perfectionism OCD focused on speaking experience intrusive doubts, thoughts, or fears regarding their verbal communication. Someone with fear of speaking OCD may find it difficult to say specific words, phrases, etc., or to express certain concepts or themes when around others.
These obsessions and compulsions can range in severity, but what causes OCD to get worse over time is not properly managing the condition earlier on. Stress, trauma, avoidance, or even something as seemingly innocuous as a change in routine can all contribute to the worsening of OCD.
At its most severe, however, OCD can impact someone's ability to work, go to school, run errands, or even care for themselves. People with severe OCD have obsessions with cleanliness and germs — washing their hands, taking showers, or cleaning their homes for hours a day.

In the United States, about 2.2% of the population will experience OCD sometime during their lives. Rates of OCD were found to be higher with women (1.8%) than men (0.5%). Childhood OCD has a stronger genetic link than adult-onset OCD, with up to 65% having a genetic link.
Obsessive-compulsive disorder (OCD) is a common, chronic, and long-lasting disorder in which a person has uncontrollable, reoccurring thoughts ("obsessions") and/or behaviors ("compulsions") that he or she feels the urge to repeat over and over.
Speaking from experience, I would say that the average uncomplicated case of OCD takes from about six to twelve months to be successfully completed. If symptoms are severe, if the person works at a slow pace, or if other problems are also present, it can take longer.
The long-term effects of OCD generally develop due to the poor quality of life that most extreme sufferers have. Long-term effects include depression, constant anxiety and an increased risk of substance abuse. It is best to get on the path to recovery as soon as possible to prevent the worsening of these effects.
How is OCD diagnosed? There's no test for OCD. A healthcare provider makes the diagnosis after asking you about your symptoms and medical and mental health history. Providers use criteria explained in the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition (DSM-V) to diagnose OCD.
Like most mental health conditions, there is no blood test or imaging study to confirm a diagnosis.
