Breaking attunement can be as easy as leaving it behind and traveling more than 100 feet away from it for 24 hours, or simply spending a short rest with that item to break the connection between the attuned and the item. If the item is cursed, however, breaking attunement is not so easy.
You can only attune to 3 items at a time, and removing attunement is not at-will nor instantaneous - a PC must spend a short rest focusing to remove attunement, spend more than 24 hours away over 100 feet away, no longer meet attunement criteria, or die.
To end an attunement a creature must spend another Short Rest with the item, similar to when they initially created the bond. Attunement to any given item can end if the creature is more than 100ft from the item for more than 24 hours. It will end if the attuned creature dies.
And, as you included in your question, attunement ends if you die. You can, of course, re-attune once you've been revived, but "being resurrected" won't cause attunement automatically.

When you unlock an Attunement after drinking a blood potion, the Attunement will be permanently select-able from the starting menu after you unlock it. You can unlock all Attunements to be select-able from the menu.

The attunement progress bar can be seen by inspecting a weapon or mousing over it in your inventory. To attune a weapon, you must use it to get kills or complete activities while it is equipped.
Practice active listening
In attempting to improve on what is attunement, you have to practice being present in the moment and listening to your partner. That means focusing on the person, watching their facial expression, feeling the emotion in their voice as they speak, and actively listening to the words.
