Anakin ultimately fulfilled his role as the Chosen One and brought balance to the Force, fulfilling Qui-Gon Jinn's belief in the prophecy that foretold Anakin's destiny. Accepting his fate, Anakin joined his Jedi mentors in death by becoming one with the Force.
A Chosen One shall come, born of no father, and through him will ultimate balance in the Force be restored." In the end, Anakin did defeat the Sith, as the prophecy foretold. His path was not obvious or direct, but his impact shaped the galaxy for the better in the end and fulfilled the prophecy.
The redeemed Anakin Skywalker accomplished the state of immortality, as well as his children, Luke Skywalker and Leia Skywalker Organa Solo, years later. Darth Sidious was able to cheat his upcoming death by transferring his spirit into a clone body while falling into the reactor core of the second Death Star.
In fact, he still was the balancing agent to the Force, allowing equal parts to both the dark and light sides, albeit coming dangerously close to rendering the Jedi extinct in the process as Darth Vader. Anakin is introduced in Episode I of the prequel trilogy, The Phantom Menace.

Of course, according to George Lucas, his six films were about the rise, fall, and redemption of Anakin Skywalker. In George's eyes, Anakin was redeemed. And of course, we see him as a Jedi spirit during the celebration at the end of the film. So clearly the Force deemed him redeemed.
Anakin would not have been able to be surpassed by Luke, Darth Vader lost a great deal of his force potential when he lost his arms and legs. Luke was (for the most part) an intact human and was the offspring of Anakin, giving him probably a bigger force potential than Vader.

Upon death, Skywalker's body disappeared and his soul became one with the Force, leaving behind only the armor of Darth Vader. Luke escaped the Death Star with his father's armor just before the battle station was destroyed by the Rebel Alliance.
Vader's mask was one of the most important pieces of Vader's armor, and was vital to his survival. Without it, he would die unless in the pressurized, controlled atmosphere of one of his specially-made meditation chambers.
A Sith Lord with the ability to prevent death and create life, Plagueis is the mentor of Sheev Palpatine/Darth Sidious, who, in accordance to the Sith's Rule of Two, eventually betrays Plagueis by murdering him in his sleep, taking his place as Sith Master.
Even in Star Wars: The Phantom Menace, after Anakin gets denied Jedi training, Qui-Gon says, "He is the Chosen One, you must see it," right in front of the young boy. That means Anakin was well aware of his status and everything that came with it.
George Lucas himself has stated that Anakin is the Chosen One and that the prophecy is true, although it had been misinterpreted by the entire Jedi Order. Lucas believes that both the dark side—greed— and the light side—compassion—are integral parts of human beings.

Anakin's new arm connected to the remainder of his limb via a synth-net neural interface, a complicated piece of technology that allowed Anakin to register feeling in his mechno-arm.
So in summary, by the time of the sequel trilogy the galaxy at large would have been aware of the truth of Darth Vader's identity (or at least the information would have been out there), but it was kept secret from the time of Vader's fall in Revenge of the Sith through six years before The Force Awakens.
“By the end of the trilogy Luke would've rebuilt much of the Jedi, and we would have the renewal of the New Republic, with Leia, Senator Organa, becoming the Supreme Chancellor in charge of everything. So she ended up being the Chosen One," the book reads.
Jedi Master Qui-Gon Jinn met Anakin while he was a slave on the desert planet Tatooine. Qui-Gon got to know Anakin and his mother, Shmi. He discovered that Anakin had no father and had the highest midi-chlorian count ever recorded. This convinced Qui-Gon that Anakin was the Chosen One.
