Other confirmed Jedi survivors are Eeth Koth, Zubain Ankonori, and Uvell. Their fates past Order 66 are either dire or unknown. It is possible that their stories could be de-canonized by future Disney+ shows, but until that happens, their names are amongst those who managed to survive the genocide of Order 66.

Aayla continued to lead troops until the final days of the war. During a mission on the remote world of Felucia, Bly received Order 66 from Supreme Chancellor Palpatine. He and his troops opened fire on Aayla, killing her instantly.
Master Yoda has risen above all others to claim the top spot as the most powerful Jedi of all time and the true chosen one according to IGN's audience. He won pretty handily as well, as he was victorious in 89,756 of his 95,243 battles and had a win percentage of 94.2%.

Order 65 stated that if a majority of the Senate or the Security Council declared that the Supreme Chancellor—at that time Palpatine—was unfit for duty, the Chancellor was to be arrested or, if necessary, killed.
Yoda and the High Council did not know about Order 66, but they did know something was wrong with the clones. During a battle in the Outer Rim, a clone trooper named Tup had a mental breakdown mid-battle and killed one of the Jedi generals leading the battle.
Despite Cody's friendship with Obi-Wan, he didn't hesitate when he received Order 66 from Supreme Chancellor Palpatine at the end of the Clone Wars. Obeying the Republic's commander-in-chief, Cody gave the order to fire on his Jedi General, then sent troops to see if he'd been killed.

In Cody's case, he seems to have felt an increasing sense of guilt for what he presumably believed was Obi-Wan's death. He no longer believed orders should be obeyed without question, and instead considered himself responsible for following the orders he had been given.
