Darth Vader lost his left arm and both legs when Obi-Wan Kenobi defeated him on Mustafar. After turning to the dark side of the Force, Skywalker severed Mace Windu's right hand to save Darth Sidious when Windu tried to kill the latter.
As a young Jedi Knight, Anakin lost a hand in a battle with Count Dooku, which received a robotic replacement. He later returned the favor by removing both of Dooku's hands. Eventually, he lost both his legs and his remaining arm in the climactic duel on Mustafar between both he and Obi-Wan.

Vader needed to take breaks, dipping himself in a liquid chamber where he allowed himself to meditate as we saw in a deleted scene of Rogue One.
Purpose. Darth Vader's meditation chamber was a life support pod that allowed the Sith Lord to survive for extended periods without the helmet and mask of his suit. The injuries Vader sustained during his duel against Obi-Wan Kenobi on Mustafar normally made this impossible.
In the duel, Anakin lost both of his legs, his one remaining organic arm, and suffered (possibly up to) fourth-degree burns across what was left of his body that were severe enough to damage his internal organs.

Even after his brutal scorching in the fires of Mustafar, sand remained the only natural phenomenon that Vader feared. To him, sand was a reminder that he'd failed to save his mother or Padme from death.
Yes. Triple amputation plus serious burns. That sounds a lot like what real life people have faced after explosions or anything else regarding fire. However, most people don't survive if their lungs were damaged beyond repair, which Vader's were, and that is a bit of a sticking point.
Even though Luke tried his best to protect him, Anakin accepted his fate and expressed his love for his son and daughter, Leia, whom he learned about before dying. While it seemed like Anakin had lived a long life, his choices eventually led to his demise at 45.
“You were right about me. Tell your sister… you were right.” Those were the last words Anakin Skywalker, aka Darth Vader, ever said while he was alive.
As Vader was lying down, with many lights over him, what he saw were those lights in a red/orange tint and different data around them. As his retinas were so injured they couldn't be repaired, the helmet was given optical filters to block excess light and thus protect Vader's eyes while also allowing him to see.
1 Darth Sidious
There has been no Sith, as powerful, meticulous, nor as evil as the Emperor himself, Darth Sidious. As previously mentioned, Sidious trained in the Dark Side by Darth Plagueis before Sidious killed Plagueis in his sleep, making Sidious a Sith Master.
As seen in the Obi-Wan Kenobi series, the protagonist was terrified of facing Darth Vader, mainly because it reminded him of his failure to save Anakin. Obi-Wan couldn't properly fight Vader due to his guilt, which brought out the fear in him that nearly caused his demise at Vader's hands.
