If anything the Locket may have been channeling her anti-muggle hatred and expanding on it. Dolores Umbridge complemented Voldemort's essence well so there really was no negative effect just an expansion on how Umbridge already acted.
Did Voldemort know Dolores had the locket? Yes, but Voldemort is convinced that no one knows he made Horcruxes. And that the Horcrux is safe in the cave. Besides, he would never allow anyone to wear the Slytherin locket.
