Facial features will soften
Strength and muscle mass will decrease. The skin becomes thinner and drier, and pores become smaller. Body hair will become lighter, softer, and less coarse. The body begins to redistribute fat to the hips and thighs.

Estrogen hormone therapy is a treatment option for people with gender dysphoria. It causes the development of typically feminine traits, such as breast tissue, decreased muscle mass, and a reduction in facial and body hair.
For example, shaving down the brow bone can create a softer female face. A small, shapely nose is possible with feminizing rhinoplasty. Genioplasty (chin reduction) and jawline contouring surgery can soften a square jaw. Jaw reduction surgery is perfect for creating a heart-shaped face, also known as “V-line” surgery.
How much breast development can I expect on HRT? After one year of HRT, the average increase in breast growth was 3.8 cm. When translating this to bra cup size, this meant around 50% of all participating women had less than an AAA cup after one year. Less than 5% of all women gained a cup size larger than an A cup.
More rapid feminization

Perhaps even more importantly, HRT provides significant protection against osteoporosis; however, HRT is only licensed for second-line use in the prevention of osteoporosis.
Reducing wrinkles
Whether or not the use of HRT can improve the appearance of wrinkles on sun-exposed areas of the body is equivocal. One study found that HRT was able to reduce skin slackness20 (and potentially wrinkle appearance, but this was not assessed).
In fact, a recent study found that women who used HRT had one point lower on the BMI scale and also about three pounds less of body fat compared to women who didn't use hormone replacement therapy.
Some women settle on 'no bleed' HRT straightaway, and some keep on getting breakthrough bleeds for up to six months. If bleeding episodes are getting less often and lighter, it will be presumed that the body is slowly settling into the HRT regime.
Weight gain and HRT
There's little evidence that most types of HRT make you put on weight. You may gain some weight during the menopause and as you get older, but this often happens whether you take HRT or not. Exercising regularly and eating a balanced diet should help you to manage your weight.
While many people are happy with estrogen, it's also very common to want a more dramatic feminizing effect, which may mean exploring gender-affirming surgeries. Body fat changes will be noticeable in the first two years, especially in the gynoid region. The face also changes significantly in the first year.
Estrogen appears to aid in the prevention of skin aging in several ways. This reproductive hormone prevents a decrease in skin collagen in postmenopausal women; topical and systemic estrogen therapy can increase the skin collagen content and therefore maintain skin thickness.
