Kokushibo was loyal to Muzan, acting as enforcer to the Twelve Kizuki. In return, Muzan sees him as a business partner, possibly due to his fanatic loyalty to him, being his strongest Demon and because the two were jointly responsible for almost exterminating Sun Breathing.
He was so afraid of death that he allowed himself to become a Demon and therefore immortal. This makes him one of the few known Demon Slayers to defect and turn to become a Demon. In doing so he abandoned a wife and two children, though unlike many new demons he did not slaughter the vestiges of his human family.
Muzan is considerably stronger than Kokushibo, yet Kokushibo was strong and loyal enough to the point where Muzan considered him a “business partner”. He held the title of Upper Moon 1 ever since the Twelve Kizuki were established.
It is revealed that Muzan's true fear of dying and immense desire to live and survive was due to him being born "dead," where his heart would constantly stop in his mother's womb and was assumed stillborn until he fought to breathe before his cremation.
While both of Yoriichi's fights are some of the most influential in Demon Slayer, the greatest Demon Slayer of all time was unfortunately unable to kill both Muzan and Kokushibo. While Muzan legitimately beat Yoriichi by escaping and regenerating, it's hard to argue Kokushibo truly won against his twin brother.
The warrior Muzan feared was a swordsman named Yoriichi Tsugikuni. Yoriichi was the only slayer before Tanjiro who came close to killing Muzan, and the flashback of their fight occurred in Chapter 187.

The only character who was stronger than Muzan was Yoriichi. He almost beat Muzan before splitting his body into 1800 parts in an attempt to escape.
Nezuko can be ranked at the level of an Upper Moon 5 since she is considerably weaker than the likes of Kokushibo and Doma.
Seeing that he still thought highly of him even after his transformation, Kokushibo couldn't help but shed a tear. Unable to truly sever his bond with his brother, he decided to keep the flute.

Kokushibo's character was quite contradictory. Even with the amount of times he felt jealous of his brother, or how he stated he hated his brother. He still held on to the precious flute that he gave to Yoriichi, because that was the only thing left that truly mattered to him.
Tsukito, Yorogi, Kuraku, and Isatsu all agree that they are loyal to Muzan.
Ever since he was in his mother's womb, Muzan was extremely frail and weak. The menace of death constantly haunted him: his heart stopped more than a few times when he was a developing fetus, and he was declared a stillborn upon birth due to having no pulse or breath.

Despite being a blood-thristy demon who has murdered several people, he has a set of principles he adheres to. One thing that Demon Slayer fans have noticed is that Akaza refuses to eat or kill women. Additionally, the Demon King, Muzan, gave Akaza special permission to do so.
Shinobu Kocho's tragic backstory is one of the most heart-wrenching in Demon Slayer. She and her sister, Kanae, were orphaned at a young age after their parents were killed by a demon.
