Here's the scoop: The first number on the size tag refers to the neck size and the second number is the sleeve length. So a shirt size “15 34/35” means that the neck is 15 inches and the sleeve is between 34 and 35 inches.
Take your favourite shirt and measure the length from the centre of the button to the centre of the button hole on the collar, this will give you your collar size in inches. It takes less than a minute and ensures you get the right size, first time.
Neck size is the measurement around your neck without ease. Collar size has length built-in to it to allow you to breathe, and for the collar to sit on your neck properly.
Similarly, size 42 means, the length of the collar is 42 cm. A size 40 shirt from one brand may have 42 inches chest length, and for another brand, it may have 44 inches chest length.
Neck sizing works the brass less than full length sizing. By neck sizing, you keep from working the body of the case and in some situations may get an extra firing before needing to trim. But eventually you will have to full length size so you can extract the brass without beating on the bolt handle.
You should be able to fit 1-2 fingers between the collar and your neck when it is buttoned. You should not have a large gap showing between the collar and neck. Heavier men should make sure their necks are not puffing out over the top of the collar.

Most adult male neck sizes range between 14 - 19 inches (35.5 - 48.3 cm), with the average neck circumference being around 15 inches. Like excess stomach fat, having a larger neck size is not good for your health.
And research has also shown that neck circumference is also correlated with BMI and sleep apnea. Men with a neck circumference above 17 inches and women with a neck circumference above 15 inches also have an increased risk of developing or having obstructive sleep apnea.
To test that you have made the right choice, try sliding two fingers in between your neck and collar without having to force them in. If you can't, it's too tight. Alternatively, if the collar doesn't touch your neck at all, and you can put your fingers in without moving the cloth, it's too big.
You can instantly tell if a shirt fits by looking at the shoulder seams. The seams should line up with the point of your shoulder bones, as close as possible. If the seams fall off your shoulders and into your bicep, it's too big. If the seams fall on your traps towards your neck, that's a sign it's too small.
Probably a large if you are thin and an x-large if you are a bit bigger. I'm 6ft and 180 pounds and I can wear both but a xl for a looser fit. In proper mens outfitters the size of shirt would be chosen to suit your build and your neck size.
