The Peaky Blinders Grace death may have been a turning point for Thomas Shelby but that doesn't mean there couldn't be a way for him to come back from it. Ever since losing Grace, Tommy has had hallucinations of her suggesting it is the one thing that haunts him more than anything else he has done.
Finally, he does not betray her and brings her son back for her.
But without McCrory, the show acted swiftly in deciding Polly's fate. In the opening of Peaky Blinders' sixth season, it's revealed that Polly has been killed by the very same people who ruined Tommy's assassination plan against Oswald Mosley (Sam Claflin) in the season 5 finale.
However, he pursues and seduces her until things have begun to develop between them. Finally, Polly decides to come for him at night and sleeps with him. Does Thomas know Grace betrayed him? Polly and Thomas realize that Grace betrayed them.

Instead, the creator decided that killing her off was the more honorable thing to do and the death is revealed early on in the new season. “We wanted to pay tribute to the character and hopefully the actor at the same time, so we wanted that to be part of the show.”
Why didn't Polly forgive Tommy? Polly had Michael taken away from her many years ago, hence why she doesn't want to let him get deeper involved in the business. She doesn't want to let him go because he is the only thing she has, and if she were to lose him she would not forgive Thomas.
She was eventually sent to Australia where she supposedly died of "Spring Fever," a mosquito-borne illness with symptoms akin to influenza and arthritis. Her body's buried there. In the end, the decision was made to kill off the character and this was done at the season's premiere. Aunt Polly was collateral damage from Tommy Shelby's attempted assassination of Sir Oswald Mosley (Sam Claflin) which was thwarted by the IRA.

Season 5. Alfie is revealed to be alive, living in Margate. His face is visibly scarred from Tommy's shooting, leaving him blind in his left eye. Alfie is acutely aware of the rise of fascist politician Sir Oswald Mosley (Sam Claflin), as well as Tommy's career in politics.
Tommy's wife, Lizzie Shelby (Natasha O'Keeffe) ends up leaving him after he has an affair and as the pair split, Charlie, Tommy's son with Grace, decides to go with her. Given Tommy feels he is dying, he lets go, feeling that the boy would be taken care of.
