His unbalanced rage allows the calmer Ventress to play with the Jedi Knight, ultimately landing a face blow with her saber that produces Anakin's scar. The pain of the scar, while not posing much of a threat to Anakin, does reveal how far to the Dark Side he is slipping.
Origins of the Scar
In the comic, Anakin fights a lightsaber duel with Count Dooku's apprentice, Asajj Ventress. She eventually gets the upper hand but decides to show off her precision. Instead of killing him, she swipes the tip of her lightsaber over his right eye.

He is plagued by nightmares of her and visions of her fate. When he eventually saves her from a group of Tuskens on Tatooine, his blind anger at her dying state consumes him. Focused on revenge as he murders an entire Indigenous tribe, Anakin has begun the path to the Dark Side.
Consumed by hate, Anakin lashed out against Padmé with the Force, gravely wounding her. Obi-Wan and Anakin dueled with lightsabers across the molten landscape. Kenobi bested Anakin, severing his limbs, and leaving him to die, burning on the shores of a lava river.
Palpatine began abusing Anakin right from the word “go”, and from the moment he clapped eyes on the kid in The Phantom Menace. What he did at first was known as “grooming”. Now, some people don't fully understand what grooming entails, and the process of how Palpatine groomed Anakin.
In the duel, Anakin lost both of his legs, his one remaining organic arm, and suffered (possibly up to) fourth-degree burns across what was left of his body that were severe enough to damage his internal organs.
However, when they next met in Star Wars: Episode II – Attack of the Clones, Anakin and Padmé's feelings for one another grew into something resembling love, and their wedding at the end of the film solidified their relationship as one of the most important romances in Star Wars history.
Letting her go just might have saved her life and the Jedi Order. So, clearly, Anakin never actually loved Padmé. He was simply afraid of being alone and losing the one that loved him, and that is only greed, attachment and selfishness, not love. Yoda was right all along: Anakin's fear led him to the Dark Side.

Originally a slave on Tatooine, Anakin Skywalker is a Jedi prophesied to bring balance to the Force. He is lured to the dark side of the Force by Chancellor Palpatine (Darth Sidious) and becomes a Sith Lord, assuming the name of Darth Vader.
Bail Organa was present during the climactic end of Star Wars: Revenge of the Sith, and there can be no doubt he was aware of Darth Vader's true identity.
As an addendum to what Infantry said: Sidious didn't ask to the med droids to put anesthesia in Anakin on purpose. He wanted him awake in order to feel the pain during the whole procedure, since pain is part of the Sith philosophy of life.
While George Lucas has never officially commented on it, it's agreed upon that when someone's eyes turn yellow, it means they are fully immersed in the dark side of the Force. They've lost control of emotions such as hatred, letting the dark side take over momentarily.
As a young Jedi Knight, Anakin lost a hand in a battle with Count Dooku, which received a robotic replacement. He later returned the favor by removing both of Dooku's hands. Eventually, he lost both his legs and his remaining arm in the climactic duel on Mustafar between both he and Obi-Wan.
Anakin does not have even the slightest bit of remorse. By the end, he feels nothing but anger. Although he feels regret for killing Padme, he still helps the evil Emperor. The film ends with Anakins standing side by side with the man on whose behalf he has committed all his crimes.
What Padmé knew about Anakin's previous dark deeds. Padmé was well aware that Anakin had committed dark crimes that would've started his fall to the dark side. He even confessed to her that he had massacred an entire Tusken Raider village to avenge his mother, leaving no one alive.
This is confirmed later in the movie when Obi-Wan informs Padme that Anakin killed them. Both are torn apart with hurt and confusion, trying to make sense of something so terrible.
At least partially, Padmé is experiencing a kind of Stockholm Syndrome or more specifically, the idea of equating affection with crime, a disorder known as hybristophilia.
The marriage, witnessed by the droids C-3PO and R2-D2, was kept secret due to Amidala's place as a prominent Senator in the Galactic Republic and because of Skywalker's membership in the Jedi Order; the Jedi Code prevented Jedi from falling in love and forming attachments such as marriage.
This time jump makes Padmé a very mature 24 years old, while Anakin is still a Jedi Padawan and teenager at the age of 19. This formidable time in their lives is when the romance between the two sparks, so the age difference between Padmé and Anakin doesn't hold them back.
Vader's suit is unbearably itchy. He also has needles stuck in him all the time. The Dark Lord has claustrophobia due to his suit. The iconic "I am your father" line was initially kept secret, even from the actors.
If he were to take his mask off, the Sith Lord would soon suffocate. So it's the most crucial part of his armor. He can only breathe without the need for his mask when he is in one of his meditation chambers seen in The Empire Strikes Back.
Purpose. Darth Vader's meditation chamber was a life support pod that allowed the Sith Lord to survive for extended periods without the helmet and mask of his suit. The injuries Vader sustained during his duel against Obi-Wan Kenobi on Mustafar normally made this impossible.
