During his second encounter with Muzan Kibutsuji, he injures his right eye in the Infinity Castle. As the battle progresses, Tanjiro suddenly collapses and his wound begins to form a large mass over his right eye due to the poison inflicted on him by Muzan.
Not one to back off from a fight nor is he one to overlook injustice; he has fought and slain several demons, but his battle with Daki pushed him over the edge, and with unbridled rage boiling within him, Tanjiro's eyes became bloody.
It was after Daki unleashed an attack and harmed multiple people, Tanjiro was extremely angry at this that his eyes were turning blood-shot red.
Chapter 204 of the series takes place three months after the battle with Muzan, and gives us an update on the extent of Tanjiro's injuries. Although his demonic transformation physically restored his right eye and left arm, now that he's cured those body parts have been left unusable.
As Tanjiro pushed past the threshold of pain during this fight, he almost managed to kill Daki. The fuming rage that Tanjiro felt from seeing Daki destroy human lives helped him push past his limits while also making him bleed through his eyes in anger, resulting in his eye colors changing to bloodshot red.
Moreover, any Nichirin sword can become bright red regardless of the blade's original color. However, the blade can only stay bright red for a certain amount of time before returning to normal.
Tanjiro certainly did a lot of good with his Water Breathing techniques, but his ability to use Sun Breathing both gives him an edge in battle and allows him to stand out among the incredibly powerful Hashira he fights alongside.
After returning to his human form, Tanjiro loses the ability to see from his right eye, the iris of which is now a dull pink-gray color and the pupil black rather than its original white.
Superhuman Eyesight - Kanao possesses extremely sharp eyesight, being able to accurately predict her opponent's next attack and movements by simply observing the subtle shifts of their muscles, joints and the slight movements of their eyes.
The last user is Tanjiro Kamado, who learned the Breathing Style from his father, Tanjuro Kamado. Other people can learn this technique if taught by a Kamado descendant.
In the case of Tanjiro, the mark used to be just a graze, but during his fight with the Hand Demon during the Final Selection, the mark changed. That life threatening condition altered him and afterwards, it develops into a flame-like pattern and is his new Demon Slayer Mark.
Advanced Demon Form: Nezuko has the ability to enter a berserk state where she becomes more demonic in appearance, transforming into an adult version of herself with vine-like tattoos that circle her body, a single horn protruding from her right forehead side.
Not only did this mark burn into his skin, but his hair changed in the moment as well to become far more fiery. This further implies that this is connected to the Sun Breathing techniques as soon after Tanjiro was able to push further with one final Hinokami Kagura boosted strike.
Even 400 years later, Muzan's fear of Yoriichi remained fully intact, as simply laying eyes on Tanjiro's hanafuda earrings caused him to recall their clash.
During his battle with Muzan Kibutsuji, he loses his right arm.

In the Anime
These earrings that Tanjiro wears have an extensive history. They were originally worn by the legendary Demon Slayer Yoriichi Tsugikuni, the originator of the breathing styles that so pervade the Corps. Tanjiro's ancestor was a good friend of Yoriichi and arguably was one of his only true friends.
The Thirteenth Form of the Sun Breathing Style is unique in the sense that it doesn't really have a name of its own. Rather, it's a combination of all the twelve forms present in this style, used in succession to maximize their power. This move was devised by Tanjiro solely to defeat Muzan.
