To block caller ID for an individual call, dial 1831 (or #31# from a mobile) then the number you are calling. Generally, this will make sure the person you are calling will not see your number.
If the Show My Caller ID switch in iPhone Settings is grayed out and unusable, that means your carrier doesn't support hiding caller ID via this method. Try hiding your caller ID by using a special block code or contact your carrier. Make sure to check the FAQs if this doesn't work for you.

The way that your name is able to show up on the receiving phone's caller ID is by that phone's carrier pulling the record in real time and displaying it on the phone during an active call. So, the receiving phone's carrier does a “dip” to Neustar and pulls the CNAM record when you call.
