The traditional method of treating gum recession is through a gum graft, which entails donor tissue being harvested from another area of the mouth—usually the palate—and transplanted onto the receding area to cover the exposed tooth root.
Receding gums around the crown can indicate a bruxism habit, be a sign of gum disease, or an ill-fitting crown. Inflammation, pain, and odd smells can all be signs of tooth decay under the crown or gum disease. If your crown is loose, damaged, or has fallen out, you need to seek dental care as soon as possible.
The method used depends on the cause and its extent, but the two main treatment techniques for addressing gum recession are either by gum veneers (removable plastic veneer which skirts around the crown margins Fig 1) or by gum graft surgery (along with some contouring of tooth edges Fig 2).
It's never too late for gum grafting as long as the patient has sufficient gum tissue to harvest for the graft. However, the success rate of the procedure may be lower in cases where the patient has advanced gum disease or other underlying health issues that can affect the healing process.
Your dentist will clean above and below the gum to remove bacteria and plaque. Then a regenerative material – a membrane, tissue-stimulating protein or graft tissue, will be applied to encourage your body to naturally restore the damaged bone and tissue. The gum is then securely reattached over the root of the tooth.
Throughout your surgery, you can expect to feel no pain whatsoever, as the anesthesia will ensure you are thoroughly numb. However, once you are home and the effects have worn off, it is not uncommon to experience some soreness and discomfort. Fortunately, there are ways to manage the pain and avoid complications.
The cost of gum graft surgery will depend on how much area is affected and how much the gums have receded. On average, a gum graft can cost between $600 and $1,200 for a small area around one tooth. Dental insurance often covers at least part of the cost of a gum graft.
Getting gum grafts can restore the way your gums look, giving you a healthy-looking smile. They will also protect your gums from bacteria and acid attacks. If your teeth had become sensitive because the roots were exposed and a cavity had started, the gum graft surgery should stop the sensitivity.
Pinhole Surgical Technique
Next, your periodontist uses special instruments to loosen your gums and move them back to the appropriate position. This procedure has gained popularity due to it being far less invasive than traditional gum grafting, which in turn makes the recovery time significantly faster.
Pinhole technique is another option that offers an alternative to surgery for gum disease treatment. It is a minimally invasive treatment that helps address gum recession. Pinhole technique involves making small holes in your gum line, pinholes.
It is not uncommon for tooth decay to develop beneath older crowns due to a breakdown in the bond or sealant after years of biting or chewing food. Dr. Knight will remove the existing crown to clean out the current tooth decay, replacing it with a new one once the tooth is fully healed and healthy again.
If only the gum is removed and not the bone, the gum tissue will grow right back after about 8 weeks, negating the purpose of the crown lengthening.
You can have a perfectly healthy mouth – no infection, no periodontal (gum) disease and no active tooth decay – and you can be very conscientious about practicing good oral hygiene, but you may still experience gum recession. Several factors contribute to gum recession.
Additionally, the teeth that will be covered during grafting need to be healthy, as infected teeth can develop further infection if they are subjected to grafting. Certain health conditions like autoimmune diseases and diabetes could also take you out of the running for a gum grafting procedure.
Gum graft surgery is a safe and effective treatment that will remarkably improve the patient's oral health. Patients will be able to return home on the same day. But if sedatives were used, they will need to arrange for transportation. The tissue healing typically takes about four to eight weeks.
As most surgical procedures, gum grafting surgery implicates risk of infection, bleeding, and reaction to anesthesia. Besides, severe pain, bone rejection, inflammation, swelling, nerve injury, bacterial exposure of wounds, loose sutures, uneven healing, and more…
It's the recommended course of treatment for receding gums to avoid exposing the teeth' root surface that leads to decay and sensitivity. Gum grafts help reverse these adverse effects and restore gums' health so that they can maintain their protective seal around the teeth.
Most patients will be fully healed from a gum graft procedure within two weeks but could take up to a month. A follow-up appointment with the doctor a week after the procedure will be needed to ensure the graft was successful and patient is properly healing. After two weeks, regular flossing and brushing can resume.
Gum graft surgery is recommended when the gums have receded enough to expose the roots of your teeth. As the gums recede, they form pockets between the tooth and the gums, which become a breeding ground for bacteria buildup.
If you use your own tissue, the most teeth you can typically graft at once would probably be somewhere in the neighborhood of 7-8 teeth. With alloderm, you could graft every single tooth in your mouth in one sitting!
If you don't get your gum disease treated, it can cause serious problems. Over time, the bacteria in plaque will start to attack your gums, bones and even your teeth. This can lead to infection, inflammation and even tooth loss.
While a gum graft procedure isn't considered major surgery, it does require some recovery time and short-term modifications to your diet.
