Anti Wrinkle injections placed along the jawline at the base of the muscle responsible for the downward pull can lift a downturned mouth. Dermal Fillers can also be used in this area to treat the Marionette Hollows associated with a downturned mouth.
Downturned Mouth Filler, also known as Marrionette Line Filler is becoming more and more popular among patients who are wanting to reverse signs of lower facial ageing. The treatment targets the long, vertical wrinkles that develop from the corners of the lips to the chin, otherwise known as marionette lines.
To lift and support the corners of the mouth you only need a very small amount of filler. The way I do it is kind of a backward scale injection technique where I go under the corner at an angle in an upward direction and then down at an angle to actually push those corners of the lip up.
How does dermal filler (Juvederm) help? If the corners of your mouth turn down or droop, fillers can help add to support and structure to uplift the mouth and restore your smile back to its natural position. Think of it as the filler acting as invisible scaffolding supporting and lifting the corners of your mouth!
Noninvasive techniques to rejuvenate perioral tissues or lift the mouth corners include injecting fillers, Botox, or a combination of these. Fillers around the mouth corners have been used to provide structure and support in this area to lift the corners of the mouth.
Corners of the mouth: 3-6 units. Crow's feet (outer canthal lines): 5-15 units. Eyebrow lift: 2-5 units. Forehead lines / horizontal lines: 10-30 units.
Botox for a corner of the mouth lift can correct a common issue in a matter of minutes without surgery, incisions, or downtime. Genetics and gravity can cause men and women to experience downturned corners of the mouth that make them look angry or upset.
Age and gravity can cause the mouth to turn downwards. In addition, the muscles responsible for tooth grinding can also cause premature drooping at the corners of the mouth. Fortunately, there is a safe and non-surgical solution to this common aesthetic concern– Botox.
Drooping mouth corners, drooping mouth or sagging mouth, happens as part of the aging process. The skin loses elasticity and the amount of fat under the skin decreases. This will cause the appearance of a permanent look of unhappiness around the mouth. Even when smiling, the corners will droop.
Both Juvederm and Radiesse are effective in treating marionette lines, but Juvederm recommended for shallow marionette lines. Radiesse recommended for more severe marionette lines and wrinkles. Juvederm works by smoothing parentheses lines, and other superficial to deep lines and wrinkles.
Wait up to a week before worrying about your weird smile. The Bad: If a week has passed and you still experience a swollen “Joker” smile, you may have been injected with too much product. Try massaging the area to breakdown the product naturally, or talk to your doctor or injector about dissolving the product.
Botox® or Dysport® are a great way to address purse lines, a gummy smile, and a downturned mouth. We would usually recommend a combination of dermal fillers and Botox to smooth out laugh lines and marionette lines.
In cases where dermal fillers have produced an undesired or harmful result, a reversal agent called Hyaluronidase is injected into the skin to dissolve the filler. Hyaluronidase is a naturally occurring enzyme in the body that works to break down Hyaluronic acid, supporting its rapid and constant turnover.
TREATMENT OF DOWNTURNED MOUTH
Small injections using a very fine needle are performed into the muscles which cause the mouth corners to pull down . Typically about 2 injections will be performed.
The DAO muscle is the muscle that pulls the mouth corner down. When you inject Botox into the DAO muscle, the muscle atrophy's and ceases to pull your mouth downwards (if you are curious about how botox works in the muscle, read our cartoon on the topic to help you understand) Hence the mouth corner lift Botox.
On the day of the procedure, your medical provider injects 3 to 6 units of Botox in each corner of your lower lip. The targeted muscle is called Depressor Anguli Oris, and it's responsible for pulling the corners of your mouth downward. The Botox temporarily paralyzes the muscle, lifting the corners of the mouth.
Marionette lines ( from the sides of the mouth to the chin) typically require 0.5cc – 1cc per side. In order to reverse the turning down of the oral commissures in the corners of the mouth you could easily use 0.5cc-1.0cc in just this area alone.
Botox relaxes the depressor anguli oris muscle, responsible for the downturn allowing for a more relaxed expression. Botox smooths the facial folds and lifts the corners of the mouth. The number of units in this area can range from four to five units per side.

Juvéderm for Marionette Lines? To treat the vertical lines between the mouth and chin to reduce the look of sagging, you're likely to need one syringe per side.

Typically 1-2mls at least are required. Some patients will require more than this.
Bellafill patients have reported results lasting upwards of 7 years or even longer, whereas the results of other fillers vary from 6 months to 3 years. Bellafill is especially good for treating marionette lines, as well as other wrinkles and scars.
