Each nipple has 15 to 20 openings for milk to flow. When your baby nurses, the action of baby's jaw and tongue pressing down on the milk sinuses creates suction. This causes the milk to flow out of your breast and into your baby's mouth.
Your milk comes from many holes, not just the obvious one
In reality, milk comes from many openings in the nipple. Called milk duct orifices, these tiny holes usually number from around four to twenty per breast.
Sometimes a woman's breasts make milk even though she is not pregnant or breastfeeding. This condition is called galactorrhea (say: guh-lack-tuh-ree-ah). The milk may come from one or both breasts. It may leak on its own or only when the breasts are touched.

Colostrum: your first milk
The fluid your breasts produce in the first few days after birth is called colostrum. It's thick and usually a golden yellow colour. It's a very concentrated food, so your baby will only need a small amount, about a teaspoonful, at each feed.
Despite views to the contrary, breasts are never truly empty. Milk is actually produced nonstop—before, during, and after feedings—so there's no need to wait between feedings for your breasts to refill. In fact, a long gap between feedings actually signals your breasts to make less, not more, milk.
The first few days: Your breast milk coming in
The hormones will get you on track with starting to produce milk. Around day three after your baby's birth, your breast milk 'comes in' and your breasts may start to feel noticeably firmer and fuller.
“Breasts increase two to three times in size during lactation,” says Ross. Hormonal changes, primarily caused by prolactin, make the breasts engorged with milk production. And if you already had breast stretch marks and prominent veins caused by pregnancy, Ross says they may intensify during breastfeeding changes.

Montgomery glands are the main white spots that become more visible due to pregnancy and hormone changes. Montgomery glands are present on both the nipple and the surrounding areola. They contain an oily substance that keeps the nipples soft and supple.
Nipple: Your nipple contains several tiny pores (up to about 20) that secrete milk. Nerves on your nipple respond to suckling (either by a baby, your hands or a breast pump). This stimulation tells your brain to release milk from the alveoli through the milk ducts and out of your nipple.
Pain while breastfeeding is usually down to sore, tender nipples, especially once your milk 'comes in' around two to four days after giving birth. Your baby will be feeding every couple of hours, which means the problem can worsen quickly, with some mums finding their nipples crack, bleed or become blistered. Ouch!
How much expressed milk should I give my baby? Every baby is different. Research shows that in babies aged one to six months, one baby may take as little as 50 ml during a feed while another may take as much as 230 ml. Start by preparing a bottle with around 60 ml, and see if your baby needs more or less.
