Pushing your baby out You may not feel the urge to push immediately. If you have had an epidural, you may not feel an urge to push at all. If you're having your 1st baby, this pushing stage should last no longer than 3 hours. If you've had a baby before, it should take no more than 2 hours.
A C-section is major surgery. The procedure can increase complications for the mother and raise the risk during future pregnancies. Women giving birth for the first time should be allowed to push for at least three hours, the guidelines say. And if epidural anesthesia is used, they can push even longer.
For first-time mothers the average length of pushing is one-to-two hours. In some instances, pushing can last longer than two hours if mother and baby are tolerating it. Normally, the baby is born with his face looking toward mother's back (referred to as an anterior position).
But if you're close to 10 centimeters dilated the research suggests it's not an issue. Evidence suggests it's more of a theoretical fear that just adds additional stress and in some cases results in an epidural (or a higher dose of epidural) to mask that urge.
Doctors have to wait until the cervix is at least 4 centimeters dilated before doing an epidural. Otherwise, the epidural will slow the process down too much. However, once the cervix becomes fully dilated it is too late for an epidural to be given.
An epidural can generally be performed at any stage; it is never too late. This is, however, not the case if baby's head is crowning (within the birth canal).
The active phase
Labor pain may radiate around your abdomen, back, and thighs. Your cervix is also dilated around 4 to 7 centimeters. Someone who has never given birth before may be in active labor for around three to six hours; a person who's done it before might take one to three hours.
Until recently, women have been asked to start pushing as soon as the cervix has dilated to 10 centimeters, but as long as you do not have a fever and your baby's heart rate is normal, there are many benefits to waiting to push until you feel the need to push.
The cervix must be 100% effaced and 10 cm dilated before a vaginal delivery.
Your cervix needs to open about 10cm for your baby to pass through it. This is what's called being fully dilated. In a 1st pregnancy, the time from the start of established labour to being fully dilated is usually 8 to 18 hours. It's often quicker (around 5 to 12 hours), in a 2nd or 3rd pregnancy.
The most common reason for telling a women not to push is that her cervix is not fully dilated. Often when a baby is in an occipito posterior position the woman will feel the urge to push before the cervix is completely open.
Purple pushing, coached pushing, holding your breath, all mean basically the same thing. Mothers being instructed on pushing causes them to hold their breath and push down into their bottom. Another more normal and less exhausting option would be “breathing or bearing down” working with the contractions.
For first-time moms, it can last from 12 to 19 hours. It may be shorter (about 14 hours) for moms who've already had children. It's when contractions become strong and regular enough to cause your cervix to dilate (open) and thin out (efface).
Uterine contractions naturally help to push out this blood and put pressure on the blood vessels in the uterus in order to stop the bleeding. For this reason, your care team will perform fundal massages whether you have a vaginal or C-section delivery.
How long does an average C-section take? Usually, a cesarean takes about 30-45 minutes.
When your baby is ready to begin the journey through the birth canal, your cervix dilates from fully closed to 10 centimeters. This process can take hours, days, or even weeks. But once you hit active labor – about 6 cm dilated – it's usually just a matter of hours before you reach full dilation.
Aside from pooping, moving through the different stages of labor may bring a whole host of other bodily fluids, from pee to amniotic fluid. During labor, you may pee a lot, especially if you get an IV at the hospital.
During active labor, your cervix will dilate from 6 centimeters (cm) to 10 cm. Your contractions will become stronger, closer together and regular. Your legs might cramp, and you might feel nauseated. You might feel your water break — if it hasn't already — and experience increasing pressure in your back.
Discourage more than three to four pushing efforts with each contraction and more than 6 to 8 seconds of each pushing effort (avoid counting to 10 with each pushing effort). Take steps to maintain a reassuring fetal heart rate (FHR) pattern while pushing.
Although it is the shortest phase, the transition phase is the most challenging. Transition typically lasts 30 minutes to 2 hours as your cervix fully dilates from 8 cm to 10 cm.
Most pregnant people begin pushing once their cervix dilates (widens) to about 10 centimeters. This is the beginning of the second stage of labor, when your baby starts moving out of your birth canal.
Labour may take longer and be more likely to involve complications. There's an increased risk of having a forceps or vacuum-assisted delivery or a caesarean, and of birth injury to the mother or baby. There is a link between fetal macrosomia and shoulder dystocia.
A fully dilated cervix is 10 centimeters open.
This is about the size of a bagel, and means that when your cervix is measured with two fingers, they can be stretched 10 centimeters across.
The average labor lasts 12 to 24 hours for a first birth and is typically shorter (eight to 10 hours) for other births.
The nurse will communicate with your doctor or midwife. Active labor begins once you are dilated to 6cm. You may be sent home if you are less than 6cm and you and your baby are healthy.
