The cervix must be 100% effaced and 10 cm dilated before a vaginal delivery. The first stage of labor and birth occurs when you begin to feel persistent contractions.
The second, pushing phase of labor continues after the cervix is fully dilated (open) to 10 cm until the delivery of your baby. It averages 4 to 8 hours but can be as short as several minutes.
Until recently, women have been asked to start pushing as soon as the cervix has dilated to 10 centimeters, but as long as you do not have a fever and your baby's heart rate is normal, there are many benefits to waiting to push until you feel the need to push.
When your baby is ready to begin the journey through the birth canal, your cervix dilates from fully closed to 10 centimeters. This process can take hours, days, or even weeks. But once you hit active labor – about 6 cm dilated – it's usually just a matter of hours before you reach full dilation.
Stage 2: Pushing and Birth
The second stage of labor begins once you are fully dilated to 10 cm. Your provider will let you know that it is time to start pushing your baby out. This stage can be as short as 20 minutes or as long as a few hours.
If your water (aka “amniotic sac,” “bag of waters” or “membranes”) hasn't broken on its own when you arrive at the hospital, and you're five or more centimeters dilated, your OB might recommend bursting the bag by hand—especially if your cervix seems to be making slow (or no) progress.

Getting up and moving around may help speed dilation by increasing blood flow. Walking around the room, doing simple movements in bed or chair, or even changing positions may encourage dilation. This is because the weight of the baby applies pressure to the cervix.
Try being upright
One of your biggest allies is gravity. When you are upright—standing, sitting or kneeling—the weight of your baby presses on the cervix, encouraging it to open. An upright position may also help get your baby into the best position for birth.
While being told you're dilated toward the end of your pregnancy is exciting, keep in mind that it doesn't necessarily mean labor is imminent. You can walk around for weeks with your cervix at 1 cm, or go from zero to 10 cm over the course of one day.
Stage two: full dilation and pushing
Once the cervix has reached 10 cm, it is time to push the baby out. Contractions continue but also produce a strong urge to push. This urge might feel like an intense need to have a bowel movement.
But if you're close to 10 centimeters dilated the research suggests it's not an issue. Evidence suggests it's more of a theoretical fear that just adds additional stress and in some cases results in an epidural (or a higher dose of epidural) to mask that urge.
Some women will instinctively push before their cervix is fully dilated. This is often treated as a complication, and a common approach is to encourage the woman to stop pushing due to fear that cervical damage will occur. However, there is no evidence to support this concern.
Laboring down is the process of not actively pushing once the second stage of labor and intense contractions begin. Some people wait one to two hours before pushing, which allows the baby to naturally move down the birth canal. Laboring down has risks and benefits.
Pushing sooner during childbirth is just as safe for most women and babies as pushing later, researchers report. The best time to start pushing during labor has been a matter of debate. Many U.S. hospitals recommend delaying pushing, but evidence has been inconclusive.
There's no scientific hard and fast rule for how long the latent and active phases last in women. The active stage of labor can range from a woman dilating anywhere from 0.5 cm per hour up to 0.7 cm per hour. How fast your cervix dilates will also depend on if it's your first baby or not.
Theoretically, the unequal walking pattern created by the curb causes the pelvis to open and allows the baby's head to descend. When the baby's head is deeper into the pelvis, there is more pressure on the cervix, causing dilation and effacement. This uneven walk should be done for about 10 minutes.
The cervix generally needs to be dilated to 10 centimeters before it's ready for the baby to pass through. Your cervix can be dilated to a couple of centimeters for a few weeks before delivery. This softening can cause the mucus plug to be dislodged and come out.

Can you feel your cervix dilating? As your cervix starts to thin and soften, you may or may not notice twinges and sensations in that area of your pelvis. This can be as much you trying to convince yourself something is happening though!
Early dilation often feels like menstrual cramps as the cervical changes cause pain and cramping noticed in the lower part of the uterus. It is the same sensation and location as menstrual cramps. Active labor tends to be felt in a larger area but can be a similar sensation as cramping (with more intensity of course).
Most women will feel increased pressure in their perineum, rectum, and low back at this stage. For many women, the rectal pressure feels the same as having a bowel movement. As the baby's head begins to appear, you may feel a stretching or burning sensation.
Your body goes through exactly the same process of labour regardless of your baby's size. In fact, the size of your baby has no influence whatsoever on the first stage of labour. Your cervix doesn't know there's a 4.5kg (9.9lb) baby sitting on top of it. It has to fully dilate regardless!
The most common description of the level of pain experienced was extreme menstrual cramps (45 percent), while 16 percent said it was like bad back pain and 15 percent compared it to a broken bone.
