Treatment should be applied twice, a week apart, in cases of scabies. Itching may last for 2 to 3 weeks after full treatment. Use an anti-itch cream or tablets from your doctor or pharmacist, if needed. Do not be tempted to apply further anti scabies cream as this may aggravate the irritation.
Permethrin is the drug of choice for the treatment of scabies. Topical permethrin should be administered every 2-3 days for 1-2 weeks to treat crusted scabies. Benzyl benzoate 25% (with or without tea tree oil) Benzyl benzoate may be used as an alternative topical agent to permethrin.
Permethrin lotion is usually applied to the skin in one or two treatments, but occasionally three treatments are necessary. If live mites are seen two weeks (14 days) after the first treatment with permethrin cream, then a second treatment should be applied.
Some people need to treat two or three times to get rid of the mites. Be sure to see your dermatologist for treatment. You should never use a scabicide used to treat crops or livestock. People who develop crusted scabies, also known as Norwegian scabies, often need repeat treatments to get rid of the mites.
How long it takes to get rid of scabies. You or your child can go back to work or school 24 hours after the first treatment. Although the treatment kills the scabies mites quickly, the itching can carry on for a few weeks.
Everyone who has scabies needs 2 treatments, one week apart. Everyone who has had skin contact with someone with scabies for more than 5-10 minutes, e.g. partner, boyfriend, girlfriend, children, household contacts etc, need to be treated. Even those with no rash or itch, should have at least 1 treatment.

But how do I know if I have cleared the scabies? Scabies goes away very well when the treatments and cleaning steps are followed carefully. But, if you, your child or anyone in the house is still getting new bumps two weeks after the last treatment, that person needs to be checked again.
Scabies will not go away without treatment 1. First have a warm bath or shower. 2. Then cover the whole body with cream/lotion, from the chin down to the soles of the feet, in between the fingers, under the nails and on the private parts.
Leaving the cream on for too long can result in skin irritation, itching, and redness. To prevent this, wash the cream off after the recommended amount of time. In the next section, I will provide more information on how to use permethrin cream for scabies treatment.
Why? The rash and itching of scabies can persist for several weeks to a month after treatment, even if the treatment was successful and all the mites and eggs have been killed. Your health care provider may prescribe additional medication to relieve itching if it is severe.
These symptoms may be a temporary reaction to the remains of the mites. This does not mean this cream did not work or that it needs to be reapplied. If you feel that the itching and rash is intense or if it continues beyond 4 weeks, talk to your doctor or health care professional right away.
Leave permethrin on the body for 8 to 12 hours
It's best to apply it just before going to bed and leave it on overnight.
Do not use in larger or smaller amounts or for longer than recommended. You may have a temporary increase in itching, swelling, or redness of treated skin when you first start using permethrin topical.
Our study showed that there was no resistance to permethrin. We attribute the patients' failure to respond to treatment for two main reasons. First, the patient did not correctly apply the treatment. Second, family members did not receive treatment.
You can get reinfected with scabies if you are exposed again after treatment.
After treatment (8 hours for cream, 24 hours for lotion) you can bath or shower as normal. You can return to work or school. You will not give scabies to anyone.
However, you do need to wash your bed and pillow sheets in hot water. This should be done the morning after applying the medication at night. You should not shower until you have put the sheets in the wash.
Wear light clothing while permethrin cream is on your skin. Do not let your skin touch anyone else's skin while wearing the cream. After 8 to 14 hours, fully wash cream from skin using warm, soapy water. Wash all bedding, towels, and recently worn clothes in hot water or have them dry cleaned.
Notably, scabies can live for 48-72 hours on a mattress. It is also important to note that scabies can still live for up to 72 hours or three days, even without human contact.
If itching still is present more than 2 to 4 weeks after treatment or if new burrows or pimple-like rash lesions continue to appear, retreatment may be necessary. Skin sores that become infected should be treated with an appropriate antibiotic prescribed by a doctor.
In infants and young children, common sites of scabies usually include the: Fingers. Face, scalp and neck.
For the treatment of scabies (5% cream): Read package directions carefully before using. Thoroughly wash and dry skin. Massage the cream into the skin from the head to the soles of the feet, paying special attention to creases in the skin, hands, feet, between fingers and toes, underarms, and groin.
If you have scabies, pay special attention to the areas where mite burrows can commonly occur - the front of your wrists and elbows, beneath your breasts, your armpits, and around the nipples in women. Leave the cream on for 8-12 hours for scabies, or 12 hours (overnight) for crab lice.
The activity of this family of mites in the skin creates an allergic reaction that causes the itching. This allergic itching may last for a week or so after treatment, even though the scabies mites are dead. You can get medicine from your family doctor that will help stop the itching.
