It's very important to change a tampon every four to eight hours. It's rare, but if you leave a tampon in for too long, you may be in danger of Toxic Shock Syndrome (TSS). If you're going to be sleeping for more than eight hours, it's a better idea to wear a pad to bed.
No matter how light your flow is, or even if there is no flow, bacteria can build up. Changing your pad every 3 or 4 hours (more if your period is heavy) is good hygiene and helps prevent bad odors.

In fact, we asked a few experts about the best way to sleep when you're on your period, and they unanimously agreed on one position: the fetal position. "Sleeping in the fetal position takes pressure off the abdominal muscles," explains Lisa Lindley, M.D., board-certified gynecologist with Eisenhower Women's Health.

It is hard to define normalcy of number of pads per day. On total, one to seven normal sized pads or tampons per period are normal.
Over time, free bleeding also has a positive effect on the period itself. By menstruating without the use of strange prodcuts inside the body, menstruation is given back its naturalness. Less menstrual pain and a shortened menstrual period, for example, are health benefits of free bleeding.
Early treatment is especially important for TSS. If you have any concern that you might have TSS while using a tampon, you should remove your tampon, get immediate medical attention, and tell the healthcare provider that you're worried you might have TSS.
"TSS is a rare but serious bacterial illness. In the U.S., TSS is estimated to affect 3-6 people per 100,000 per year," said Erin Clark, MD, an obstetrician-gynecologist with University of Utah Health.
If you notice it only when wiping or can easily absorb any light bleeding with a pantyliner (or even just underwear), it's spotting. If you need a pad or tampon to absorb bleeding between periods, it generally wouldn't be considered spotting.
