It's up to you. Women usually remove their bras to allow the massage therapist to work on the back and shoulder area without getting massage oil or lotion on the bra. If your problem area is your low back, hips, buttocks, or groin, tight-fitting or large underwear can sometimes get in the way of massage work.
OPTION #1: Leave it all covered
It is possible to receive a massage fully clothed. Your massage therapist can perform your massage service regardless of attire. While they will still be able to work on your body, you may miss out on the thoroughness that comes when your therapist addresses the whole muscle, end-to-end.

Most forms of massage therapy do not include these areas. The massage therapist should not touch areas that you have explicitly requested that they avoid. Your breasts and groin area should not be exposed during the massage.
A 60 minute full body massage typically starts with a head & scalp massage to get you relaxed. Then, they will work the neck and shoulders, move on to each arm, and work their way to the legs & feet. Next, you will flip around on the massage table and they will work on the back of the legs and potentially the glutes.
If an at-risk situation occurs, you can refer the client to another practitioner, seek professional or legal advice, or contact your preferred Massage Association or insurer for advice.
If your are male then you will usually be asked to disrobe to underwear. If it is only the upper body that needs treating then you can leave your trousers on. If you are female and are having a general treatment then you will also be asked to disrobe to underwear and will be covered by towels the whole treatment.
A therapist is still able to perform a high quality massage even if their client is asleep. It's not rude, nor does it insult them in anyway.
