Psychosis in bipolar disorder (BD) is characterized by the presence of either delusions or hallucinations or both[1]. It is well known that over half of the patients with BD develop psychotic symptoms during their lifetimes[2,3]. Psychotic symptoms are more frequent in bipolar than in unipolar depression[3-5].
People with psychosis typically experience delusions (false beliefs, for example, that people on television are sending them special messages or that others are trying to hurt them) and hallucinations (seeing or hearing things that others do not, such as hearing voices telling them to do something or criticizing them).
Having moments of psychosis (when people lose some contact with reality) means experiencing: delusions – thoughts or beliefs that are unlikely to be true. hallucinations – hearing and, in some cases, feeling, smelling, seeing or tasting things that are not there; hearing voices is a common hallucination.
Some patients may do well on lithium or divalproex plus an atypical antipsychotic; others may need triple therapy consisting of lithium, divalproex, and an atypical antipsychotic. Atypical antipsychotic are also recommended for patients who present with psychotic features (e.g., delusions, hallucinations).

Bipolar people can be abusive, but then, so can non-bipolar people. The above notwithstanding, the extreme mood shifts of bipolar disorder may sometimes have a disinhibiting effect on abusive impulses that would otherwise not get expressed.
Patients with rapid changes between the two states are known to have manic depressive states or episodes. Without the appropriate treatment, bipolar behavior can destroy relationships, deteriorate the individual's health, and/or endanger their job.
