In the Return of the Jedi, Luke was seen dragging Vader's body to the docking Bay.
Luke told C-3PO he would be right along, but first lit fire to the armor in the hopes that the mistakes of Darth Vader would be shed and that the name of Anakin Skywalker would become a light to guide the new Jedi for generations.
In the aftermath of the slaughter, Vader dueled his former Jedi Master, Obi-Wan Kenobi. Vader lost the fight after having both legs and his left arm severed by Kenobi. Vader then tumbled down the volcanic planet's surface, near a river of lava, where his robes burst into flames, consuming his entire body.

The majority of Anakin's/Vader's body suffered a rather fiery engulfment after his battle with Obi-Wan on the Mustafar system. It left him nothing more than a limbless charred torso on the edge of a lava bank, only to be stuffed into an uncomfortable suit to torment him for the rest of his life.

His lungs were so charred, his retinas so scarred, and his face so deformed he would forever need a mask to breathe and speak properly. Implanted in his chest is an annunciator linked to a processor that allows his damaged vocal cords to produce speech.
Vader's suit took care of most of his need, which also included food. The suit feed Vader nutrients, which gave him the energy he needed to continue throughout his days. He also consumed RepMed vita paste through a straw, which was located within the grills of his mask.

Darth Vader lost his left arm and both legs when Obi-Wan Kenobi defeated him on Mustafar. After turning to the dark side of the Force, Skywalker severed Mace Windu's right hand to save Darth Sidious when Windu tried to kill the latter.

In the duel, Anakin lost both of his legs, his one remaining organic arm, and suffered (possibly up to) fourth-degree burns across what was left of his body that were severe enough to damage his internal organs.
