A sebum plug can look like a tiny bump under the surface of the skin or it may stick out through the skin like a grain of sand. When a sebum plug forms, bacteria that normally lives harmlessly on the surface of your skin can start to grow within the follicle.
At first glance, keratin plugs may look like small pimples. They are usually pink or skin-colored. They also tend to form in groups on specific parts of the body. However, keratin plugs don't have the noticeable heads that typical pimples might have.
If you find your spots on the scalp are sebum plugs, they help to keep the hair shafts healthy, and thus the whole of your hair. They protect the hair and skin against bacteria, too, so it is important not to try to remove them, simply because they are there.
If you were to squeeze a sebaceous filament, a white or yellow worm-like structure may ooze out. Or, the filament may not produce anything. Always take caution as trying to extract sebaceous filaments can injure the skin and cause permanent scarring. It can also damage and stretch the pore, making it appear bigger.
Sebum plugs result from hormones and increased stress levels. Sebum plugs usually develop on the face, including the forehead, chin and nose. They look more like pimples and have a “head.” In fact, sebum plugs appear before zits and can turn into pimples.
Though you may be tempted to squeeze or otherwise get rid of a sebaceous filament, it's best to leave them alone. Squeezing or picking at sebaceous filaments risks scarring and spreading any bacteria that may be in or around the pore to other parts of your face, causing a breakout.
Nazarian recommends exfoliating with topical medications, such as glycolic acid, retinoids, and salicylic acid, to break down the plugs and dissolve them. Eventually, your pores will refill, so like a game of Whac-a-Mole, those sebaceous filaments will pop right back up, requiring you to be consistent in your routine.
Usually you can spot a blackhead easy enough, sebum plugs are a little trickier but if you grab a magnifying glass you can see them no problem. You will notice that although they may look like little black dots from a distance, they are actually more of a white or yellow color.
Due to an overgrowth of oil-producing cells, sebum can become trapped inside the gland, causing it to swell and form a bump under the skin. This is known as sebaceous hyperplasia. These bumps are harmless and often appear on the forehead and cheeks.
Sebum plugs commonly form on the forehead and chin. And because nose pores tend to be large, when they become even partially clogged, plugs can be even more noticeable. Plugs can also appear on your upper arms, upper back, or just about anywhere you have hair follicles.
They occur when the pore's walls break down, causing a bigger pimple to form. Pustules are typically white and, unlike papules, they contain pus. A person may find that a pustule is tender to the touch.
The lipid-laden cells in the sebaceous glands are wholly secreted (holocrine secretion) to form sebum. Triglycerides compose the majority of the lipid found in sebaceous gland cells. From the sebaceous glands, sebum drains into the hair follicle (see Fig. 2.11), from which it exits onto the surface of the skin.
Follicular keratotic plugs are a marker of DLE and were originally described as a sign of early and active lesions and not in areas of scarring or healed skin. [3] They correlate with the hyperkeratosis and the plugging of follicular ostia with keratotic material.
Keratosis pilaris develops when keratin forms a scaly plug that blocks the opening of the hair follicle. Usually plugs form in many hair follicles, causing patches of rough, bumpy skin. Keratosis pilaris is caused by the buildup of keratin — a hard protein that protects skin from harmful substances and infection.

Pimple pus is made from sebum (oil) that gets trapped in your pores, along with a combination of dead skin cells, debris (such as makeup), and bacteria.
Those tiny white or yellow bumps under the skin are not worrisome, but they might get frustrating. Milia develop under the skin when bits of dead skin cells, made up of proteins called keratin become trapped and create really hard, little white balls. Milia are essentially harmless and usually don't need treated.
Hard pimples are caused when dead skin cells, oil, and bacteria get under the skin's surface. Certain types of hard pimples should be treated by a doctor to prevent them from getting worse and leaving scars.
A clogged pore can look like a black spot, called a blackhead, or it can look like a white or skin-colored raised bump called a whitehead. A pimple or acne cyst, on the other hand, has more bacteria and inflammation inside of them and can have pus, redness, and swelling.
"Salicylic acid is an excellent comedolytic, or pore-clearing, ingredient because it exfoliates the stratum corner (the surface of the skin) and penetrates into pores to remove sebum." Clogged pores can look enlarged, bumpy, or, in the case of blackheads, dark in color. The more oil that a person's skin produces, the more likely it is that their pores will become blocked. A person can use skin care techniques and products to manage or clear clogged pores.
Prolonged exposure to the air causes the blockage to oxidize and darken. Because the pore gets stretched out of shape, it will fill back up again even after it is emptied. These stubborn pores are most likely to occur on the face, chest and back.
The pores on our face, just like our ear canals, are designed to clean themselves. So for most people, leaving them to their own devices is fine, and just cleansing the face is enough.
