In A Breath of Snow and Ashes, Ian and Emily miscarry twice before their daughter, Iseabail, is born, only to die shortly thereafter.
Sadly, Emily loses the baby and the experience guts Ian. But that doesn't stop him from showering Emily with even more devotion. When Emily loses a second child, things change. Since he isn't full-blood Mohawk, Ian is told that his spirit is "doing battle" with Emily's spirit.
In late spring 1779, Ian returns to the Fraser's Ridge in North Carolina, bringing his pregnant wife Rachel with him. Their first son is born in early June 1779.
We eventually learn that Ian had two unborn children who Emily (his wife) miscarried and then a daughter Yeksa'a Iseabaìl, who dies as a baby.
Ian married Janet "Jenny" Fraser, sister to Jamie Fraser, in 1740 shortly after her brother was imprisoned and her father died. Ian had returned from war having lost part of his leg; Jenny came up to him while he was working and kissed him; and told him that they would get married.

Later, after Claire has returned to the Ridge, she is confessor to Allan Christie, who explains that he was the one that fathered Malva's child, and then murdered her.
He was clearly depressed. And while out surveying the Ridge's land with Roger, Ian came close to attempting suicide with Claire's Water Hemlock. Roger stopped him before he could ingest it in a tea. Ian eventually reveals he got married while living with the Mohawk.
However, Young Ian turned to Claire with a confession. The baby could be his. It turns out Ian and Malva did sleep together.

Ian returns in Season 10 with Mickey and both are eventually paroled. Ian struggles with life after prison, building a future with Mickey and his own insecurities, but eventually he and Mickey get engaged and are married.
Harding, who just turned 36, announced to his social media followers that he and his wife Sophie Hart have welcomed a baby.
The young Malva then later claimed Jamie was the father of her baby, and was eventually found dead in the Fraser's vegetable patch. In a particularly harrowing scene, Claire tries to save the unborn baby but is tragically unable to.
1. "Faith," (Season 2, Episode 7) While it's easy to love Claire and Jamie when they're in love, it's another thing to see Balfe's character reveal she “hated” Jamie for the events leading up to her tragic miscarriage of their first daughter.
Fanny leaves the house with Claire while Jamie attends to the man's final wish. She mostly stays in the barn with the goats, staying away from the house and making no effort to help bury her husband. When the time comes to leave, she gathers her goats and leaves the trading post with the Frasers.
Faced with the demons of his past when he is unexpectedly reunited with his Mohawk brother Kaheroton (Braeden Clarke), Ian opens up to his uncle Jamie (Sam Heughan) about his love story with his Mohawk wife, Wahionhaweh (Morgan Holmstrom), who he referred to as Emily and their children lost to miscarriages.
Following the Battle of Culloden, Fergus continues to live at Lallybroch, and Jenny Murray tutors him with the rest of her children. He sometimes brings food to Jamie, who lives in a cave near the estate.
As a result Ian made no attempts to try and wed Lizzie. Lizzie was betrothed to Manfred McGillivray for a while. However he called the engagement off when he realized he had contracted "the French disease".
When Jamie's negotiations with the governor for Claire's freedom fail, Tom Christie confesses to Malva's murder. In November 1776, Allan comes to Malva's grave and confesses to Claire about raping his sister, that he was the father of Malva's child, and that he killed her because she was going to tell the truth.
The haircutting was part of the process. Malva wanted to make Claire undesirable. She wanted to make Jamie turn to her instead of to his wife. We can get a sense of that from the way Claire worried about Jamie seeing her with the short, jagged hair.
Claire eventually realizes that Malva is lying because she knows her husband well enough. Tom seems to question Malva's story. You can see it in his face that he's trying to figure out if this story is true. And then there are the reactions at the funeral when Allan refuses to let Claire carry the baby's coffin out.
Jenny meets Brianna Randall Fraser for the first time at Lallybroch, when the girl travels into the 18th century to search for her parents. Jenny is delighted at seeing that Jamie fathered a daughter as they had never expected him to have children, and marvels at Brianna likeness to him.
Although Claire couldn't exactly tell her the truth, Jenny's feelings are as justified as she said she would have liked to have known she was alive. Jenny also adds that she knew Claire was hiding something from her ("First Wife"). It will be another 10 years after this before Claire tells the Murrays the truth.
Since there is a price on Jamie's head, he, his sister Jenny, and his brother-in-law Ian concoct a scheme wherein she "turns him in" to the British, as he is wanted for the crime of treason for the part he had played in Bonnie Prince Charlie's attempted Scottish uprising.
