Patients with alcoholic hepatitis will have scleral icterus and jaundice as well as tender hepatomegaly with or without ascites and if their hepatitis is severe will have asterixis and exhibit mental confusion on examination.
Generally, symptoms of alcoholic liver disease include abdominal pain and tenderness, dry mouth and increased thirst, fatigue, jaundice (which is yellowing of the skin), loss of appetite, and nausea. Your skin may look abnormally dark or light. Your feet or hands may look red.
Alcoholic Hepatitis
The predominance of AST over ALT in alcohol-related liver disease was first reported by Harinasuta et al. in 1967. Many authors have since described AST/ALT ratios greater than 1.5 or greater than 2.0 as being highly suggestive of alcoholic hepatitis.
An AST/ALT ratio higher than one (where the AST is higher than ALT) means you may have cirrhosis. An AST/ALT ratio higher than 2:1 (where the AST is more than twice as high as the ALT) is a sign of alcoholic liver disease.
Alcoholic liver disease is defined by three stages of liver damage following chronic heavy alcohol consumption: fatty liver, alcoholic hepatitis, and fibrosis/cirrhosis (Figure 5).
It is estimated that alcohol-related fatty liver disease develops in 90% of people who drink more than 40g of alcohol (or four units) per day. That's roughly the equivalent of two medium (175ml) glasses of 12% ABV wine, or less than two pints of regular strength (4% ABV) beer.
However, people who are having these tests should refrain from drinking alcohol for 24 hours before the test.
The liver damage associated with mild alcoholic hepatitis is usually reversible if you stop drinking permanently. Severe alcoholic hepatitis, however, is a serious and life-threatening illness.
After abstinence from alcohol for 2 to 3 weeks, hepatic steatosis completely resolves and liver biopsies appear normal when examined by electron microscopy.
These results indicate that microheterogeneity of glycoproteins, serum prealbumin level and erythrocyte ALDH activity are good markers of alcohol abuse, and serum GDH/OCT ratio is the most sensitive marker of alcoholic liver injury.
The AST is typically in the 100 to 200 IU/L range, even in severe disease, and the ALT level may be normal, even in severe cases. The AST level is higher than the ALT level, and the ratio is greater than 2:1 in 70% of patients. A ratio greater than 3 is strongly indicative of alcoholic hepatitis.
Drinking a large amount of alcohol, even for just a few days, can lead to a build-up of fats in the liver. This is called alcoholic fatty liver disease, and is the first stage of ARLD. Fatty liver disease rarely causes any symptoms, but it's an important warning sign that you're drinking at a harmful level.
Blood tests
A low level of serum albumin suggests your liver is not functioning properly. A blood test may also look for signs of abnormal blood clotting, which can indicate significant liver damage.
Bilirubin. Bilirubin is a substance produced during the normal breakdown of red blood cells. Bilirubin passes through the liver and is excreted in stool. Elevated levels of bilirubin (jaundice) might indicate liver damage or disease or certain types of anemia.
The bottom line. Just like a broken bone or infection needs time to heal, so does an overworked liver. While this depends on the amount of alcohol you have had over the years, your liver can see partial healing within two to three weeks, but this will depend on your health history.

People in the early stage of the disease may live between nine and 12 years, while people in the late stages may only live two years. You can help extend your life expectancy by treating the underlying cause of your condition. If you drink, quit.
To check your blood for alcohol, your doctor uses a needle to take blood from your arm and measure the amount of alcohol. The other tests you might get for alcohol, like a breath or urine test, don't use blood samples. Each of these tests has the same goal: to check how much alcohol is in your body.
Carbohydrate-deficient transferrin (CDT): CDT helps doctors identify heavy alcohol use. Increased levels of CDT suggest that a person may be consuming more than 50 to 80 grams of alcohol, roughly equivalent to 3 to 6 drinks, per day for two to three weeks.
After two weeks off alcohol, you will continue to reap the benefits of better sleep and hydration. As alcohol is an irritant to the stomach lining, after a fortnight you will also see a reduction in symptoms such as reflux where the stomach acid burns your throat.
Heavy drinkers and alcoholics may progress from fatty liver to alcoholic hepatitis to cirrhosis, and it is estimated that 10 percent to 15 percent of alcoholics will develop cirrhosis.

Having 2 to 3 alcoholic drinks every day or binge drinking can harm your liver. Binge drinking is when you drink more than 4 or 5 drinks in a row.
