A coma doesn't usually last longer than several weeks. People who are unconscious for a longer time might transition to a lasting vegetative state, known as a persistent vegetative state, or brain death.
Severe brain injury is usually defined as being a condition where the patient has been in an unconscious state for 6 hours or more, or a post-traumatic amnesia of 24 hours or more. These patients are likely to be hospitalised and receive rehabilitation once the acute phase has passed.
Recovery from coma is a gradual process, starting with the person's eyes opening, then responding to pain, and then responding to speech.
A coma rarely lasts beyond two to four weeks. A coma may be the result of several conditions, including: Traumatic head injury—A severe traumatic brain injury. Stroke—A lack of or interrupted blood flow to the brain.
Severe injuries can cause permanent unconsciousness. Of people with severe injuries, 60% to 80% survive. Many of those who survive are completely unconscious for some period.

Some examples of early responses to watch for are: Localized response: These are appropriate movements by the patient in response to sound, touch, or sight. Turning toward a sound, pulling away from something uncomfortable, or following movement with the eyes are examples.
Often before death, people will lapse into an unconscious or coma-like state and become completely unresponsive. This is a very deep state of unconsciousness in which a person cannot be aroused, will not open their eyes, or will be unable to communicate or respond to touch.
A person who is brain dead is dead, with no chance of revival. Coma: A state of profound unresponsiveness as a result of severe illness or brain injury. Patients in a coma do not open their eyes or speak, and they do not exhibit purposeful behaviors. Some patients need ventilators while others do not.
Most people do come out of a coma
In some rare cases, a person might stay in a coma for several weeks, months or even years. Depending on what caused the person to go into a coma, some patients are able to return to their normal lives after leaving the hospital.
A vegetative state is similar to a coma but isn't the same. People in a vegetative state have recovered enough that they aren't in a coma, but their brain's abilities and activity are still very limited. The potential for recovery from a vegetative state varies widely.
Post-traumatic amnesia typically occurs after a survivor wakes up from a coma, a period of unconsciousness. During this time the survivor may not be able to recall certain memories. This can last from a few minutes to a couple weeks, depending on the severity of the brain injury.
The levels of response in the components of the Glasgow Coma Scale are 'scored' from 1, for no response, up to normal values of 4 (Eye-opening response) 5 ( Verbal response) and 6 (Motor response) The total Coma Score thus has values between three and 15, three being the worst and 15 being the highest.
Level 4: confused - agitated. As the brain improves, it begins to “wake up” and may have difficulty controlling the level of response to the environment. This is called “agitation.” You will see the patient will have poor memory and be confused most of the day.
Pediatric brain injuries are classified by severity using the same scoring levels as adults, i.e. 8 or lower reflecting the most severe, 9-12 being a moderate injury and 13-15 indicating a mild TBI.
Nursing and other medical staff usually talk to sedated people and tell them what is happening as they may be able to hear even if they can't respond. Some people had only vague memories whilst under sedation. They'd heard voices but couldn't remember the conversations or the people involved.
Brain death (also known as brain stem death) is when a person on an artificial life support machine no longer has any brain functions. This means they will not regain consciousness or be able to breathe without support. A person who is brain dead is legally confirmed as dead.
As many as 15 to 20 percent of patients who appear to be in a coma or other unresponsive state show these inner signs of awareness when evaluated with advanced brain-imaging methods or sophisticated monitoring of electrical activity.
People who do wake up from a coma usually come round gradually. They may be very agitated and confused to begin with. Some people will make a full recovery and be completely unaffected by the coma. Others will have disabilities caused by the damage to their brain.
Some patients awaken from coma (that is, open the eyes) but remain unresponsive (that is, only showing reflex movements without response to command). This syndrome has been coined vegetative state.
Because patients who are in a coma can't eat or drink on their own, they receive nutrients and liquids through a vein or feeding tube so that they don't starve or dehydrate. Coma patients may also receive electrolytes -- salt and other substances that help regulate body processes.
Speaking may not affect their clinical outcome; time spent with them takes time away from other, more "viable" patients. Comatose patients may, however, hear; many have normal brain-stem auditory evoked responses and normal physiologic responses to auditory stimuli.
Patients in a coma might have brain stem responses, spontaneous breathing and/or non-purposeful motor responses. Coma has three possible outcomes: progression to brain death, recovery of consciousness, or evolution to a state of chronically depressed consciousness, such as a vegetative state.
