The cumulative incidence of wait-list mortality in frail vs nonfrail candidates at 6 months was 8.0% (23 of 290) vs 4.0% (33 of 818), at 1 year was 16.5% (48 of 290) vs 8.4% (68 of 818), and at 3 years was 32.0% (93 of 290) vs 17.0% (139 of 818).
The waiting period for a deceased donor transplant can range from less than 30 days to more than 5 years. How long you will wait depends on how badly you need a new liver.
People needing liver or heart transplants often need to wait nine or more months. Recipients are assessed for compatibility to the donor (not just blood type, but for six different tissue antigen subtypes as well as general body size – e.g. putting an adult heart into a small child is not possible).
Overall, infections are the most frequent cause of mortality in males and females, though they are significantly higher in females. In our cohort, the main causes of mortality within the first year after transplantation were infections and surgical complications in both sexes.

Those who don't recover may die within days. Chronic liver failure is a more gradual process. People can live with cirrhosis for months or years while waiting for a liver transplant, although the need becomes more urgent when complications, such as portal hypertension, begin to develop.
Liver transplantation is an ultra-major operation and probably the most difficult of all transplant operations. The hospital mortality rate after liver transplantation has ranged from 2% to 16% 1, 2, 3, 4, 5, 6, most series reporting a rate of about 10%.
Very common longer-term risks
The most common infections are chest or urine infections. These are usually fairly straightforward to treat with antibiotic tablets. Infections inside the liver transplant itself can be harder to treat. The most common of these is a virus called CMV (cytomegalovirus).
A whole liver may be transplanted, or just part of one. In most cases the healthy liver will come from an organ donor who has just died. Sometimes a healthy living person will donate part of their liver.

The thinking then — and among proponents of the practice today — was that six months of abstinence gave a patient's liver time to heal and, thus, avoid a transplant. If that didn't work, the patient would have proven they can stay sober and would not return to drinking after a transplant.
People with higher MELD scores are generally offered donated livers first. Time spent on the liver transplant waiting list is used to break ties among people with the same MELD scores and blood types. Some liver conditions, such as liver cancer, may not result in a person getting a high MELD score.

Liver failure
There is a risk that your remaining liver doesn't work after your operation. This is a rare but serious complication and can be life threatening.
People with cirrhosis of the liver have a life expectancy of between two and 12 years. If you have early-stage cirrhosis, treatment and lifestyle changes can help you live longer. People with advanced cirrhosis of the liver have a much shorter life expectancy.
Recovering from a liver transplant can be a long process, but most people will eventually be able to return to most of their normal activities and have a good quality of life. It can take up to a year to fully recover, although you'll usually be able to start gradually building up your activities after a few weeks.
Liver transplant can have excellent outcomes. Recipients have been known to live a normal life over 30 years after the operation.
One reason for this is that older adults with liver disease often have many other health challenges which make recovery from transplant surgery more difficult. However, researchers have recently reported successful liver transplants in older adults—even in people who are in their 80's.
The post-transplant survival rates for patients aged younger than 70 years were 86.6% at 1 year (95% CI, 82.4-89.9; I2=99%), 73.2% at 3 years (95% CI, 63-81.3; I2=99%), and 70.1% at 5 years (95% CI, 66.8-73.2; I2=99%).
People with liver failure, which is when the liver completely shuts down, can usually only live a day or two without treatment. In some cases, liver dialysis can support a failing liver by removing toxins from the blood.
The structure of the scar tissue has created a risk of rupture within the liver. That can cause internal bleeding and become immediately life-threatening. With respect to stage 4 cirrhosis of the liver life expectancy, roughly 43% of patients survive past 1 year.
