The Grand Inquisitor was a proficient Form II practitioner. Jedi Master Ki-Adi-Mundi was another who used the technique and also taught it to others in the Order at the Jedi Temple on Coruscant. Crossguard lightsabers were once common among Makashi practitioners, many of whom fought at the Great Scourge of Malachor.
Practitioners of Form V included Jedi Masters Aayla Secura, Adi Gallia, Plo Koon and Luke Skywalker, Jedi Knights Anakin Skywalker and Ulic Qel-Droma, and Dark Lord of the Sith Darth Bane.
Notable practitioners
Other than Lucien Draay, Meetra Surik, Obi-Wan Kenobi, and Kit Fisto, Shii-Cho had many advanced and/or specialist practitioners. In more recent years, the form was mastered by the Jedi swordmaster Cin Drallig along with the other six forms.
Kylo Ren's Lightsaber Form
The evidence isn't entirely clear, but it is most likely that he was an adherent of Form V, particularly the Djem So variant. Ren's fierce fighting style has the hallmarks of Djem So, which demands a high degree of speed and strength to execute a flurry of attacks and counterattacks.
A notable Jedi adherent was Johun Othone, who took full advantage of Form VI's inherent versatility during the pitched battles of the war.
Developed to combat blasters, Form III existed for thousands of years. One of the early practitioners of the form was Meetra Surik. Her tutor during her rehabilitation, Kreia, also knew Soresu. By the time of the New Sith Wars, the Sith Blademaster Kas'im had mastered Form III as well as the six other forms.
Jedi Masters Vrook Lamar, Zez-Kai Ell, and Kavar were all skilled in its use and were able to pass on their experience to others. Jedi Master Atris, who fell to the dark side during the First Jedi Purge, was also known to employ the form.
Anakin used the form 5, it's known a the Way of The Krayt Dragon. Form 5, also known by its two primary disciplines of Shien and Djem So, is the offensive version of form 3 and relies on strength.
Mace specialized in the seventh form of lightsaber combat, AKA Juyo or Vaapad. Windu was the only known practitioner of this form that didn't fall to the Dark Side, mainly because the style calls for an aggressive, unpredictable approach (much like Maul's, just less jumping around).
An example of a Jedi swordmaster was Mace Windu, considered as such because he was a master of all forms of lightsaber combat and due to his mastery of Vaapad, which he created.
Obi-Wan Kenobi was highly skilled in Form III. Over the course of the Clone Wars, Kenobi continued to develop his skills.
Kyle Katarn and Jedi Knight Alema Rar, among others, were trained in the use of the fourth form of lightsaber combat. Grand Master Luke Skywalker also had knowledge and skill in the use of Ataru, as did his wife, Mara Jade Skywalker.
Practitioners of Form V included Jedi Masters Aayla Secura, Adi Gallia, Plo Koon and Luke Skywalker, Jedi Knights Anakin Skywalker and Ulic Qel-Droma, and Dark Lord of the Sith Darth Bane.
It could be used to combine double-bladed lightsaber combat with other Force abilities, like pushes and lifts. Darth Maul had mastered this form and employed it in his duel against Obi-Wan Kenobi in the Theed power generator.
Sokan was a form of lightsaber combat. Developed by the ancient Jedi Knights during the Great Sith War, Sokan combined tactics that allowed for evasion and mobility with the kinetic motions of Form V combat.
Affiliation. Form VII, known by its two primary disciplines of Juyo and Vaapad; and also known as The Ferocity Form, was the seventh form of lightsaber combat. It was considered the most aggressive and unpredictable form.
Master Plo was considered to be one of the most skilled swordsman in the Jedi Order, and was capable of fending off the infamous Jedi hunter Asajj Ventress while being encumbered by a broken arm. He was also trained in the use of Form IV/Ataru.
Obi-Wan Kenobi used Form III.
Given that she studied Luke's ancient Jedi texts and was trained by Leia, Rey most likely used a variation of Form VI, Niman. Form VI, Niman, takes elements from the previous five forms and combines them into a pastiche that's proficient in all situations, but lacks the specialization of other forms.
Qui-Gon Jinn was known for using Ataru, the same form that Yoda used. This form was known for the intense acrobatics that were involved, with this one having the most complex maneuvers out of all of the forms.
As Darth Tyranus, Dooku trained both Asajj Ventress and Grievous in Form II. Makashi evidently became one of Grievous' primary fighting methods, as many of his opponents commented on Dooku's influence in his technique, and he passed this training on to his IG-100 MagnaGuards.
