A troll attacked Hermione Granger when she was crying in the bathroom after Ron made fun of her. It was later revealed that Professor Quirinus Quirrell deliberately let this troll wander into the dungeon at Hogwarts so that he could try and steal the Philosopher's Stone from Hogwarts without anybody noticing.
What's wrong with going to the bathroom? Hermione Granger was in there because she'd been crying, upset by Ron Weasley's words. She wasn't at the feast, so she didn't know about the troll.
Harry, Ron, and Hermione originally assumed that Snape was behind the "troll business", but Harry, trying to stop Voldemort in his attempt to steal the Sorcerer's Stone, found out that it was Quirrell, not Snape, who had let the troll in.

5: When Ron discovered Hermione was a girl…
Having been close friends with Hermione for the best part of four years, you would think that he may have noticed that she was a girl. But apparently, he had not. So, when he finally had that momentous epiphany, Hermione was there to quickly bring him down a peg or two.
Molly Weasley believed that Hermione was hurting Harry emotionally, and therefore Molly disliked Hermione at the time.

Harry's scar had hurt during Arrival Feast because of Quirrell's connection to Voldemort; Snape was then talking to Quirrell, allowing Voldemort, from the back of Quirrell's head, to sense Harry's presence.
It wasn't Snape that made Harry's scar burn. It was the fact that Voldemort was right underneath the turban on the back of Quirrell's head, it was his presence that made Harry's scar burn, not Snape.
