Tommy gave the sapphire to Bethany Barwell, who took it to her sister Evadne, who in turn put it around her daughter's neck. 7-year-old Connie died the next day, and according to Esme, it was Evadne that put the curse on Tommy to lose his daughter when she turns seven.
In season six, episode three 'Gold', vengeance did come. Tommy's seven-year-old daughter Ruby died of tuberculosis, after hearing voices, seeing visions and speaking the Romani words for the devil. Ruby died from a curse laid in retaliation for Tommy having passed on an already-cursed sapphire to the Barwell family.

Evadne Barwell was the mother of Connie Barwell and sister of Bethany Barwell. She was shot numerous times by Thomas Shelby, as he was avenging the death of his daughter, Ruby Shelby.
The scene shows that Polly chooses to take Michael's side in the conflict before she dies. The use of Michael's blood also implies that Polly set the curse upon Tommy, as she has previously claimed a connection to the supernatural, seemingly communing with spirits.
The final scene saw Lizzie telling Tommy that their daughter, Ruby, had fallen ill with a fever, and had been repeating the Romani words, "Tickna mora o'beng".
“Gold doesn't work, that was the last message that Ruby taught us,” Tommy says enigmatically at her funeral.
Lizzie officially reached the end of her tether after discovering Tommy had slept with Diana Mitford and decided to leave him. Still distraught over the death of her daughter Ruby, she's surprised when Tommy's son Charlie asks to join her instead of staying with him.

Lizzie and Thomas continue to grow closer following Grace's death. Thomas takes Lizzie to the canal, revealing that he used to wait there for Greta prior to France, and that he hasn't been back there since, but wanted to go back with Lizzie. With this, he kisses her, and the two have sex by the canal.

Ruby may be warning of a coming devil (Tommy, perhaps, or the fascist tide in Europe). Or, as some fans speculated, the warning is that the daughter of the devil will die. If the devil is Tommy, then the daughter is Ruby.
