Much of season four's storyline had to be adjusted due to Carpenter's real-life pregnancy; after Cordelia gives birth to Jasmine in the episode "Inside Out" she is left in a coma for the remainder of the fourth season.
As Cordelia becomes pregnant and manipulates Connor into helping her sacrifice an innocent girl for their child, his inhuman actions begin conflicting with his inner good, accelerating his already deteriorating mental health.
Tl;dr: Jasmine possessed Cordelia when she left the higher dimension; the memory restoration only triggered Jasmine's control over Cordelia; Cordelia slept with Connor in the sense that Jasmine was possessing her body at the time; Cordelia's body was pregnant with Connor's baby but it wasn't exactly a baby.
Thanks to using Cordelia's body to have sex with Connor, she became pregnant and told Connor that his new family was special.
In total, Cordelia is the victim of mystical pregnancy three times during the series: from a Haxil beast in this episode, as host of an unborn Skilosh demon in "Epiphany," and from Connor while possessed by Jasmine, as first revealed in "Salvage."
Later, in Angel's perfect-day dream sequence, Angel and Cordelia consummated their relationship, but Angel called out "Buffy!" as he lost his soul, just as he did in Sunnydale years earlier.
In her final hours, Cordelia curiously became the pseudo-villain of the series after being possessed by the malevolent entity Jasmine. Instead of getting the kind of epic, valiant sendoff that other villains-turned-scoobies did for their finales, Cordelia simply fell into a coma and died unceremoniously in a hospital.
Meanwhile, Cordelia convinces Connor to mystically expedite the birth using the blood of a virgin. Angel arrives in time to stop him, but hesitates, and Cordelia gives birth to a full grown woman.
Technically Cordelia is only supposed to be 22, but sorry Charisma, you don't look 22 in 2002 (mom hair do at fault here too, because on Veronica Mars she was back to youthful glory) And Connor was supposed to be 18 if I am correct. 4 years isn't a big deal, but these two did not look 4 years apart.
Their relationship was permanently ended roughly a year after it began, when she caught him kissing Willow. She felt hurt and stupid because of this and remained bitter toward him, Xander's snide remarks never helping.

After three seasons on Buffy, Cordelia left the series to move over to star in Angel, a spin-off series focusing on Buffy's vampire ex-lover Angel (David Boreanaz). The first season of Angel sees Cordelia move to Los Angeles, in the hopes of escaping her new-found poverty by becoming an actress.
Cordelia died in 2004, using her last moments on Earth to aid her friend and love, Angel, and put the disillusioned hero back on his heroic path. After her death, Cordelia became a higher being in the service of the Powers That Be.

Canon is that Angel loves Buffy, but due to the curse on him, they cannot be together because if truly happy, he goes back to being Angelus.
They are attacked by a Mohra demon; when Angel kills the demon he is restored to life by its powerful blood. After The Oracles — a link to The Powers That Be — confirm he is human again, Angel and Buffy spend a blissful night together.
She was emptied out, replaced. Darla was subsumed with Connor's human soul. Fred became a shell through which Illyria could be reborn. And Cordelia's entire history was erased to make room for Jasmine.

Cordelia sacrificed herself so that Mallory could rise as Supreme, going back in time and killing Michael before he could come into his own as the ender of days. But unbeknownst to the witches, another Antichrist was born to take his place—to Timothy and Emily, the lovers seen in Outpost 3.
