For a time, Masterson became Thor's new civilian identity, and the first human to wield Mjolnir on a regular basis. He quickly became known as "The Everyman Avenger."

Hence, Mjolnir determines worthiness based on Odin's definition of worthiness. However, since Mjolnir also has a conscious being within it, in the form of God Tempest, another likely explanation is that it is God Tempest that determines the worthiness.
Other Marvel characters have lifted Mjolnir not through worthiness but through technicalities such as absorbing Thor's powers, including the Air-Walker, Awesome Android, Magneto, Rogue, Wonder Man, and Doctor Doom.

In an interview with the Happy Sad Confused podcast, the Russo Brothers were asked why Captain America was able to lift Thor's hammer Mjolnir in Endgame when he seemingly couldn't in Age of Ultron. The duo replied “In our heads, he was [always] able to wield it.”
Why is this such a big deal? Well, for starters, the hammer in question, Mjolnir, can only be wielded by those who are considered “worthy,” a little mystical insurance policy put on it by Thor's dad, Odin. It's pretty cool to see the Avenger who is the purest of heart officially deemed “worthy” of the weapon of a god.

That opens the possibility that Hela retained some hold over her former weapon, enabling her to shatter it. We don't know if she bonded with Mjolnir through some sort of Asgardian mysticism, but she possessed the hammer for much longer than Thor did.

During this battle, Venom (Eddie Brock) was given the powers of Captain Universe, and was able to wield the hammer. However, Venom took this advantage a step further by merging Mjolnir with the Silver Surfer's surfboard to create an even greater weapon.

In 1974's Avengers #122 by Steve Englehart, Bob Brown, and Mike Esposito, Iron Man lifted Mjolnir in outer space, but once he neared Earth's gravity field, his hand was pinned. This proved anyone could wield Thor's hammer where there is no gravity.

Mjolnir is a hammer, and was enchanted by Thor's father, Odin, so that only those the hammer deemed "worthy" are capable of wielding or even lifting it. Stormbreaker is an axe, and although it does not have such a worthiness enchantment, its power is such that a mere mortal attempting to wield it would be driven mad.
The Powers
Accompanied by the character's own fighting techniques, Captain America is one of the strongest characters in the MCU, especially when wielding Mjolnir. However, Thor is an Asgardian, which gives him an advantage over Captain America.

With Mjolnir's worthiness enchantment resulted in Thor being able to lift the hammer only if he is considered to be worthy, but likewise, the enchantment is not discriminatory in nature, and anyone who is deemed worthy is able to lift the hammer.
One of the more surprising characters to lift Mjolnir was Deadpool. How was Deadpool ever worthy of Thor's hammer? That's because he wasn't. The hammer that Deadpool lifted was actually a fake one, forged by the trickster, Loki.
In the comics, the Hulk has been depicted as being strong enough to lift Mjolnir, Thor's hammer, on multiple occasions.
Red was part of a film crew that was transported to Asgard by Loki. And at Loki's behest, Red took Thor's Belt of Strength and bathed in the Fires of Geirrodur before claiming Thor's Iron Gloves. The gloves were the key, since they allowed Red to lift and wield Mjolnir without being worthy.
