Emotional stress, fatigue, anxiety, smoking, pain, or being cold are common causes of poor let-down. With poor let-down, you may not experience the tingling and leaking of milk that usually occurs when your milk begins to flow.
If you do not feel your milk letting down, it doesn't necessarily mean that something is wrong. You may never notice it, or you may feel it in the first few weeks then less over time. As long as you can see the signs your baby is getting enough breast milk and growing well, you don't have to worry.
It's important to know that not all moms experience these signs, and that's okay. Even if you don't have a let-down response, you can still nurse your baby adequately and pump milk without any problems.
However, it is possible for the let-down reflex to be hindered. Let-down may not be achieved for several reasons including stress, fatigue, tension, pain and even being cold. For many moms recreating the experience of cuddling with their babies while nursing can help to trigger let-down while pumping.
Increasing your milk supply will take time, so don't give up. Even dry pumps (when you pump but nothing comes out) sends a signal to your body that more milk is needed on tap, so it's getting the work done even if there's no output to show for it right away. Stick with it and you'll see the results after a few days.
You're not getting let down. If your breasts feel like they're full but you're not able to get the milk flowing out when you pump, it could be that you're not achieving let down. The let down reflex releases your milk from the milk ducts. This only occurs when you're either breastfeeding or pumping.
Pumping or expressing milk frequently between nursing sessions, and consistently when you're away from your baby, can help build your milk supply. Relax and massage. Relax, hold your baby skin-to-skin, and massage your breasts before feeding to encourage your milk to let down.
1. Feeling stressed or anxious. Stress is the No. 1 killer of breastmilk supply, especially in the first few weeks after delivery. Between lack of sleep and adjusting to the baby's schedule, rising levels of certain hormones such as cortisol can dramatically reduce your milk supply.
Oxytocin. The oxytocin reflex is also sometimes called the “letdown reflex” or the “milk ejection reflex”. Oxytocin is produced more quickly than prolactin. It makes the milk that is already in the breast flow for the current feed, and helps the baby to get the milk easily.
Avoid leaning your breast forward into your baby's mouth, as this can lead to poor attachment. Your baby needs to get a big mouthful of breast. Placing your baby with their nose level with your nipple will encourage them to open their mouth wide and attach to the breast well.
Each time baby begins to nurse the nerves in your breast send signals that release the milk in your milk ducts. This let down reflex usually happens after your baby has been sucking the breast for about two minutes. Some women feel this let-down reflex as a tingling or a warmth.
The most common cause of low milk production is that your breasts are not being emptied or stimulated enough through breastfeeding or pumping. If you're using a breast pump, you may not be pumping frequently enough or your pump flanges may not be a good fit for your breasts.
If there is a continual delay in letdown from the breast it can lead to reduced milk supply. Babies are also really in tune with how their mother's are feeling. Your baby can totally sense when you are sad, stressed, or anxious.
Your breasts feel softer
This happens as your milk supply adjusts to your baby's needs. The initial breast fullness reduces in the first few weeks. At around 6 weeks, breast fullness is completely gone and your breasts may feel soft. This is completely normal and has no effect on your milk supply.
To put a number on it, it usually takes about 20 to 30 minutes after feeding to generate enough milk for your baby, and about 60 minutes to replenish fully. The more often your baby feeds, and the more they empty your breasts, the more milk your body will produce.
Some might feed every 90 minutes, whereas others might go 2–3 hours between feedings. Newborns should not go more than about 4 hours without feeding, even overnight.
You may feel pain with plugged ducts and may also notice that your breast does not empty very well. Plugged ducts may also be seen during engorgement and mastitis. Check your breasts after every pumping. If you find your plugged ducts soon after they form, you will be able to resolve the plugged area more rapidly.
Increase pumping frequency
Generally, moms should be pumping every 3 hours. Pumping more often can help stimulate breasts to produce more milk. Moms can try pumping both breasts for 15 minutes every two hours for 48-72 hours. Then moms can return to their normal pumping routine.
Too much pumping can cause problems for nursing moms. Supply is a function of demand – the more milk that you remove, the more milk you may make. Therefore, a lot of extra pumping in addition to nursing could lead a nursing mom to have an oversupply.
Breastfeeding: The Let-Down Reflex
Let-down happens as milk is released into milk ducts in your breast. This usually happens when your baby sucks on your breast. You may even have a let-down when your baby or someone else's baby cries, or for no reason at all. Some women don't feel the let-down.
Inadequate weight gain is one of the strongest indicators that a baby is not getting enough milk. After seven days, your baby has fewer than six wet diapers and four stools per day, her urine is dark yellow or specked with red, or her stools are still dark rather than yellow and loose.
