The only reason Doma couldn't regenerate his head was because he took in poison that would kill a demon 700 times over. And the fact that he got poisoned before doesn't mean he was weaker during the fight, especially since he showed no sign of it and he has regenerative abilities.

Moreover, Doma would continually ask his subordinate to eat women, like himself, as they have more nutritional value to demons than men; Akaza vehemently refused to do so and Doma believes this was why he surpassed him in power.

At the end of the day, none of the Hashira could've defeated Akaza – even though a few of them could've succeeded in decapitating him.
The only Demon he despises the most is the Upper Rank Two: Doma. It is quite a shocker that the second most potent Demon in the Demon Slayer is not Muzan's favorite.
Absorption: Doma is able to completely absorb and devour an entire human body without eating them, having completely broken down and incorporated Shinobu Kocho's body into his own within only a few seconds after killing her.

Akaza set of techniques can overwhelm Doma during combat and Akaza has faster reaction time and speed being able to react to Tanjiro when he was in supreme territory n swung at Tanjiro like 2 times before he cut Akaza head off.
History. Doma was born with beautiful and rare rainbow colored eyes. Because of this unusual trait, his parents thought that he was able to hear the voices of God and was groomed to be the religious leader of the Eternal Paradise Cult, a position that he held until his death.
Doma says that he knew Inosuke's mother, as his cult took her in, but Inosuke denies that he has a mother since he is raised by boars.
In her final moments, she apologized to Inosuke for failing to protect him and tosses him into the river as an attempt to save his life. She was ultimately killed and eaten by Doma, who claimed to have devoured her to the bone.
It is later revealed that Doma is clinically apathetic, being completely emotionless and unable to experience proper emotions. Doma himself admitted that the concept of emotion was completely foreign to him, even as a human.
Muzan likely believed it possible that one day, Doma could become strong enough to survive sunlight, so killing him might've been detrimental to the reason Muzan made him a demon in the first place.
Even 400 years later, Muzan's fear of Yoriichi remained fully intact, as simply laying eyes on Tanjiro's hanafuda earrings caused him to recall their clash.
So the only reason Muzan is afraid of Tanjiro is because of the Hanafuda earrings he possesses, which acted as a reminder of his downfall. nxt. Hanafuda Earrings are a symbol of Sun Breathing users, traditionally passed down from generation to generation as a family heirloom.
Sound Hashira
He is the fastest Hashira, and the second physically strongest. Obviously, then, Tengen Uzui is rather proud. He is demanding, fussy, boastful, and prone to selfishness. But he is also ultimately kind and caring, with great love for his three wives as well as the other Hashira.
Gyomei can defeat Akaza without even awakening his Demon Slayer mark.
With that said, all Hashira in Demon Slayer have one glaring flaw: they're only human. As such, they're not infallible and are certainly not tougher than their demon enemies. This results in some rather lopsided matches and emotionally-charged oversights on their part.
