It's true that in later episodes, Muzan also appears as a woman wearing a kimono when he's holding court over other Demons, most of whom are also wearing traditional Japanese clothing. This might have something to do with Demons (or “Oni”) themselves being part of traditional Japanese culture.
While it is not explained in the anime why he did so, Muzan is shown to be living a normal life as a human. Not only that, he's married a woman and has a child with her.

Muzan has had more than one family over the course of his millenia-long life. This includes the birth family he had before he became a demon. In addition to them, Muzan took five wives.
He is the big bad guy (first demon) and in the manga and cosplay world, he is one of the most popular character. Also is full name is “Muzan Kibutsuji” and contains the kanji for “demon dance” (鬼き舞ぶ辻つじ kibutsuji?) and “miserable, pitiful, merciless”(無む惨ざん muzan?).
Moreover, while she was asleep, Urokodaki performed a unique hypnotism on Nezuko, making her believe that all humans are her family and she must not hurt them in any way. With her immense fondness for humanity, Nezuko broke out of Muzan's control, becoming free to do anything at her will.

What is the disease of Muzan? Muzan was born in Japan's Heian era, to a constant struggle to survive on account of his frailty. His heart stopped multiple times in his mother's womb, and when he first entered the world, he was deemed stillborn, having no pulse and not breathing.
The Blue Spider Lily medicine was also the cause behind Muzan's demonification, granting him a strong body and possibly changing his blood to have transformative properties, enabling Muzan to create more of his kind as well as hold dominion over them.
In some cases, depending on the climate, the Blue Spider Lily didn't even bloom at all. This explains why Muzan never found it, as he would have been unable to go out during the day.
