The main reason for you being tired is simply that you have had a stroke. In the early weeks and months after a stroke your body is healing and the rehabilitation process takes up a lot of energy so it is very common to feel tired.
Excessive daytime sleepiness (EDS) is a prevalent symptom among stroke survivors. This symptom is an independent risk factor for stroke and may reduce stroke survivors' quality of life, cognitive functioning, and daytime functional performance.
Regarding the duration of fatigue after stroke, acute fatigue can last up to 6 months, whereas the chronic type can persist in 40% of patients after 2 years. Another study reported fatigue to be still present in one-third of patients up to 6 years after stroke onset.
Quality sleep has many benefits, especially for stroke survivors. Getting a good night's sleep supports neuroplasticity, the brain's ability to restructure and create new neural connections in healthy parts of the brain, allowing stroke survivors to re-learn movements and functions.
For this reason, the 60 minutes after the onset of stroke symptoms are known as “the golden hour.” If treatment can be initiated within this brief window, the patient's outcome is likely to be better.
However, most providers assess progress using Brunnstrom's seven stages of stroke recovery, which include the following: flaccidity, spasticity appears, spasticity increases, spasticity decreases, complex movement combinations, spasticity disappears, and normal function returns.
Stroke impacts the brain, and the brain controls our behavior and emotions. You or your loved one may experience feelings of irritability, forgetfulness, carelessness or confusion. Feelings of anger, anxiety or depression are also common.

It takes about three months after the stroke for neuroplasticity to return to a more normal state. After that, a survivor can still work on regaining function and practice for improvement, but those improvements may come at a slower pace.”
Although just 10% of people fully recover from a stroke, 25% have only minor impairments and 40% have moderate impairments that are manageable with some special care.
The stroke recovery process is long and can come with many challenges, including the feeling that symptoms are getting worse instead of better. However, know that regression after stroke is common and often temporary. This can be impacted by factors such as new medications, schedule changes, or excess fatigue.
Fatigue effects between 39–72% of stroke survivors. Fatigue is defined as physical and mental exhaustion that is triggered by stress, medication, overwork and mental and physical illness or disease. Fatigue is largely subjective and what one patient will tolerate may be different from what another patient can tolerate.
As you begin to recover, you might feel that your behaviour changes or improves. You may start feeling better physically and emotionally. But some changes will be long term. You are still the same person, but a stroke may change the way you respond to things.
Common post-stroke physical problems include: Weakness, paralysis, and trouble with balance or coordination. Pain, numbness, or burning and tingling feelings. Fatigue, which may continue after you return home.
Changes in your emotions and to your personality are common after stroke. It's very normal to experience strong emotions after stroke, however these emotional reactions usually get better with time. Longer-term emotional and personality changes can be very challenging.
Physical therapy uses exercises to help you relearn movement and coordination skills you may have lost because of the stroke. Occupational therapy focuses on improving daily activities, such as eating, drinking, dressing, bathing, reading, and writing.
One of the most important things to do after stroke — and never stop doing until you're reached your fullest recovery — is rehab exercise. Rehab exercise helps rewire the brain and improve mobility long-term. When patients fail to do rehab exercises, their mobility may deteriorate and cause a regression.
The initial recovery following stroke is most likely due to decreased swelling of brain tissue, removal of toxins from the brain, and improvement in the circulation of blood in the brain. Cells damaged, but not beyond repair, will begin to heal and function more normally.
After six months, improvements are possible but will be much slower. Most stroke patients reach a relatively steady state at this point. For some, this means a full recovery. Others will have ongoing impairments, also called chronic stroke disease.
Because walking is such an important element of day-to-day functioning, recovering functionality in the leg is the central priority for recovering from a stroke. The arm, though, can be left to do little to nothing for the remainder of the survivor's life.
