Subsequently, the patient had been receiving a MVI tablet daily containing Vitamin B12 (1 mg) and showed improvement in his presentation [Figure 4]. Usually, hyperpigmentation resolves within 3 months of initiation of treatment, in about 87% of such patients.
Cutaneous manifestations associated with vitamin B12 deficiency are skin hyperpigmentation, vitiligo, angular stomatitis, and hair changes.
It is suggested that deficiency of vitamin B12 causes decrease in intracellular reduction potential that leads to oxidation of the reduced glutathione and decrease in GSH/GSSG ratio. The epidermal melanocytes are then stimulated to produce melanin as the tyrosinase inhibiting effect of GSH has been diminished.
Vitamin C, also known as ascorbic acid, is used as a treatment modality in depigmentation of hyperpigmented spots on the skin and gingiva.

Vitamin Deficiency
Deficiency of vitamin B-9 (folic acid) and B-12 (cobalamin) can cause pigmentation problems leading to patchy skin. Deficiencies of vitamins that occurs due to reduced intake of vegetables and fresh fruits can therefore make you skin appear dull and dark.
Studies have proved that there is a link between vitamin B12 deficiency and melasma, which means a deficiency of vitamin B12 can cause melasma on the face.
Melanocytes in the basal epidermis control skin pigmentation through synthesis of melanin, a complex process thought to be primarily regulated by alpha-melanocyte stimulating hormone (αMSH) (Figure 1—figure supplement 1A and B).
While melasma most often develops after prolonged sun exposure or during pregnancy due to fluctuating hormone levels, it may also be a sign of malnutrition or liver disease. Studies have linked melasma to people with poor liver function, vitamin B12 deficiency and iron deficiency anemia.
Hyperpigmentation occurs due to overproduction of melanin and estrogen and progesterone imbalances in the body.
For post-inflammatory hyperpigmentation (PIH), without any treatment at all it can take anywhere from three months to two years for the discoloration to fade, depending on how dark the patches are, and even then it may not disappear completely.

How long does it take for hyperpigmentation to fade? Once what's causing the dark spots or patches is found and stopped, fading can take time. A spot that is a few shades darker than your natural skin color will usually fade within 6 to 12 months. If the color lies deep in your skin, however, fading can take years.
The length of time it takes for PIH to fade depends on how dark the spot is compared to the surrounding skin. The bigger the contrast between the dark area and your natural skin tone, the longer it will take to fade. PIH doesn't always fade away on its own. In some cases, it's more or less permanent.
