There is no evidence to show that Voldemort ever knew Snape had betrayed him by revealing his plans to Dumbledore; on the contrary, there is a fair amount of circumstantial evidence to indicate that he didn't.
So he gets to know the truth about Snape minutes before he dies. But before that, he never suspected any double crossing from him, because Voldemort considered himself to be the best legilimence and believed no one could lie to him.
We know Severus Snape was "an accomplished Occlumens." We also see that Voldemort, who was a powerful Legilimens, was unable to detect when Snape was lying to him.
He convinced the dark lord that he had no idea about Voldemort being a host in Quirrell's body, and he was trying to stop Quirrell from taking the stone for himself. This is explained in a conversation between Severus Snape and Bellatrix in half blood prince.
Of course not. Up until the very end, Voldemort thought Snape was his. He quite apologised to Snape for feeling he had to kill him, calling him faithful, loyal and valuable. If he had known, Severus would have died a lot sooner than at the Battle of Hogwarts, slowly and very painfully.
Although Snape may not have projected his memory of James onto Harry, he still would have been predisposed to dislike him by virtue of him being James Potter's son. Harry being in Slytherin would not have changed this fact.
The reason, again, lies in Voldemort's arrogant confidence in his abilities as a Legilimens. He doesn't trust Snape as Dumbledore does, but he believes that he can detect any disloyalty.
Snape's fierce devotion to and love of his childhood friend Lily, Harry's mother, is the foundation of that loyalty. After Harry Potter and the Half-Blood Prince, Snape's loyalty was a matter of intense debate among the fans.
Because, for voldemort and all previous owners of the elder wand(excluding dumbledore), the wand represents ultimacy, and to gain ownership of such a powerful wand, disarming just didn't cut it.
Quirrell didn't use the Killing Curse on Harry, because Voldemort said “use the boy” to get the Sorcerer's stone from the Mirror of Erised.
Snape even said that he didn't want anyone to find out that he was protecting Lily's child, “especially Potter's son” (pg. 679 of Harry Potter and the Deathly Hallows) because of his hatred for James. Instead of providing Harry a safe haven from his abusive home with the Dursleys, he tormented Harry over an old grudge.

When Snape heard the prophecy and pleaded with Voldemort to spare Lily's life, he may have inadvertently caused his downfall. While we know Voldemort killed Lily, he did give her a choice. If she stepped aside and allowed him to murder Harry, he would spare her.
In chapter 2 of Harry Potter and the Half-Blood Prince "Spinner's End," Snape reveals that Dumbledore denied him the Defense Against the Dark Arts job because he seemed to think the position would bring about a relapse and tempt him into his old ways.
