Abstract. Empathy is a basic human ability, and patients with schizophrenia show dysfunctional empathic abilities on the behavioural and neural level. These dysfunctions may precede the onset of illness; thus, it seems mandatory to examine the empathic abilities in individuals at clinical high risk for psychosis.
Schizophrenia patients had severe empathy dysfunction based on their relative EQ ratings. There was a serious discrepancy between the self and relative/spouse assessments of the empathic skills of schizophrenia patients.
Narcissistic personality disorder (NPD) is associated with an assortment of characteristics that undermine interpersonal functioning. A lack of empathy is often cited as the primary distinguishing feature of NPD.

When it comes to the survival of intimate relationships, no matter how much love there is between you and your partner, there's no guarantee that you both will be able to empathize—even if you think you're "soulmates." Without empathy, the love in your relationship will end up like "love" as in tennis—one big zero.

People living with schizophrenia may have a distorted view of the things around them. The things they see or smell may not represent real life, and this can make normal objects scary or unusual. People with schizophrenia may also be more sensitive to light, color, and other distractions.
