We recommend a larger breast shield diameter than your actual nipple base size because when a breast pump is used it creates vacuum making your nipple larger. Having a gap of about 4mm between your nipple and the breast shield allows for your nipple to expand comfortably once you are pumping.
Nipples often expand while pumping, so it is very important to evaluate the nipple throughout the session.
A bit of swelling that goes away after a few minutes is normal due to your nipple being under constant latch (Willow 3.0, 2.0, and 1.0) and surrounded by breast milk during the pumping session. If the swelling does not resolve within 5-10 minutes, you may be using the wrong size flange.
You can expect your nipples to return to their original size and color (likely lighter and smaller than when you were breastfeeding) and extra veins should disappear, says Kasper. All those stretch marks, however, are yours to keep, she adds.
Sore nipples are a common problem with mothers that establish or maintain their milk supply by pumping. Pain may interfere with your goal of providing milk for your baby. We want to do every thing possible to help you have a comfortable pumping experience.
Your nipple should move freely in and out without touching the sides of the tunnel and without redness or pain. Only a little areola tissue should be pulled into the tunnel. You should see a bit of areola pulling into the tunnel with each pump cycle, but not too much or none at all.
The 120 minute rule is that, generally speaking, when you are exclusively pumping, you want to spend at least 120 minutes (2 hours) per day pumping. How many sessions you would spread that 120 minutes across depends on how old your baby is. With a newborn baby, you might want to do eight 15 minute sessions.
At the other end of the spectrum, increasing the milk supply too much through pumping can lead to engorgement, blocked milk ducts, and increased risk of breast inflammation, or infection (mastitis).
When your breasts are empty, they will probably feel lighter and no longer uncomfortably full, as they might have at the start of the pumping session. You can also pick them up to see if they still feel heavy or full of milk. Pumping sessions typically take about 15 to 20 minutes.

Sometimes an oversupply is created by over stimulating the breasts by doing both breastfeeding and pumping. Having an oversupply of breast milk can be uncomfortable for both a mother and infant.
Aim to spend 15 to 20 minutes hooked up to the pump to net a good amount of breast milk (some women will need 30 minutes or more with the pump, especially in the early days). Pump until the milk starts slowing down and your breasts feel well-drained.
Healthy infants who breastfeed effectively are often thought to be more efficient than the expression of milk either by hand or with an electric breast pump. Breastfed infants have been shown to remove 50% of the total volume of milk removed at a breastfeed in the first 2 min and 80% in 4 min [31].

Even with solid food introduction, the AAP recommends that babies receive breast milk for their first 12 months and beyond if a parent chooses. As such, parents who are considering decreasing pumping or weaning from the pump usually do so at about six months, when their baby has started to eat solids.

This time-span norm is in part due to the notion that more prolonged pumping will damage breast tissue, the fact that some pumps have built-in timers that stop the pump at 30 minutes, and because babies tend to eat for about 30 minutes between both breasts.

Most women find that starting in massage mode and then switching to expression mode once the milk starts flowing works well. Set the vacuum speed to the highest level that is comfortable for you, and start with the cycle set to 54.
Breast compression is a method where you put a little extra pressure on your milk glands by gently squeezing your breast. Compressions stimulate your letdown and increase the milk flow, and can be very helpful to both elicit your milk letdown and to keep the milk flowing during your pumping session.
