The doctors follow through with the lung surgery, but Will realizes he has to get away from Stella so she will have a chance to live a healthy life. He packs up and leaves the hospital. Eight months later, Stella's new lungs are working well, and she's enjoying good health.
While away from the hospital, Stella receives a text that her lung transplant is en route, which she ignores. She falls through the ice of a frozen pond as she and Will leave to go back to the hospital. She struggles but is near death when Will reaches into the water and pulls her out.
It ends with Will meeting up with Stella, who leads him to a room for a surprise dinner party, put on by Stella and Poe, Stella's best friend and a fellow CF patient. After that, Poe dies and Stella is sad because she never got to hug him. Stella decides that she is living her life too strictly.

Five Feet Apart is a book-turned-Blockbuster film that details and stories of Stella and Will, 2 teenagers with cystic fibrosis who meet during a clinical trial. Due to the 6-foot rule, the teens cannot touch or get too close to one another.
Stella was diagnosed with cystic fibrosis when she was six years old and knows these symptoms can be life-threatening.
The only issue is that Stella and Will can never actually touch — or even be within six feet of one another, due to the fear of cross-infection. A big, epic kiss (the climax of many a romantic drama) is firmly off the table.
That's why it's important for people with CF to stay at least 6 feet away from others with CF and anyone with a cold, flu, or infection. For people with CF, being close to others with the disease puts them at greater risk of getting and spreading dangerous germs and bacteria. This is called cross‐infection.
Richardson plays Stella, a girl who falls in love with a boy she literally cannot touch, thanks to what is known as the “six-foot rule,” a real medical standard that recommends that CF patients give each other a wide berth and wear face masks to avoid cross-infection.
However, after each getting lung transplants a few years later, they both died within five days of one another. However, "Five Feet Apart" is not based on reality. "The film is fiction and not based on the Pragers," a representative from CBS Films wrote in an email.
On 'Five Feet Apart' Sequel, 'All This Time'

Lung transplantation is a surgical option for people with cystic fibrosis who have advanced lung disease.
No. Cystic fibrosis is a genetic condition so even though the transplanted lungs will not have CF and will never develop it, the rest of the person's body will continue to have cystic fibrosis. This means that following a successful lung transplant, some CF treatment will still be needed.
A person cannot get CF from kissing someone who has the condition. However, people with CF are more susceptible to certain infections. As such, a person who has an infection should avoid kissing a person with CF. This can help protect the person with CF from contracting dangerous infections.
