Primary syphilis occurs 3 or 4 weeks after infection (although it can take up to 90 days for the sore to appear). Symptoms may include a single painless sore usually about a centimetre big at the site where the infection entered the body — such as on the penis, vagina, cervix, mouth or anus.
For the most part, a syphilis rash appears as pale, red spots on your chest, back, arms, legs, palms, and soles of your feet. The patches are symmetrical and tend to be non-itchy. The infection spreads through contact with the rash or sores, making the secondary stage of syphilis very contagious.
Primary syphilitic chancres are usually firm, round, small, and painless, and develop at the spot where the T. pallidum bacteria enter the body. The chancre lasts 1 to 5 weeks, and it heals without treatment.
These sores are usually (but not always) firm, round, and painless. Symptoms of secondary syphilis include skin rash, swollen lymph nodes, and fever. The signs and symptoms of primary and secondary syphilis can be mild, and they might not be noticed. During the latent stage, there are no signs or symptoms.
If you have syphilis, you could have a chancre. This is a hard sore or an ulcer that usually forms where the bacteria enters your body, most often in your genital area, but also in your mouth or anus. A chancre usually doesn't cause pain.
The rash often forms over the body, often on the palms of the hands and the soles of the feet. The rash usually consists of reddish brown, small, solid, flat or raised skin sores that are less than 2 cm (0.8 in.) across. But the rash may look like other more common skin problems.
In the first stage of syphilis, a small, smooth sore develops on your genitals, mouth or lips. It may resemble a pimple and be so small and harmless that you don't even notice. This sore goes away on its own in about six weeks. In the second stage of syphilis, a rough, red or brown rash develops.
A syphilis sore (called a chancre) pops up — that sore is where the syphilis infection entered your body. Chancres are usually firm, round, and painless, or sometimes open and wet. There's often only 1 sore, but you may have more.
You can get syphilis when your mouth, genitals, or another part of your body touches a syphilis sore on a person who has the disease. If you are pregnant, you can pass syphilis on to your baby even if you don't know you are infected. Can I get syphilis by having oral sex? Yes.
Symptoms of syphilis include: small sores (ulcers) on your penis, vagina, or around your bottom (anus) – these are usually painless and you may only have one of them. sores in other areas, including in your mouth or on your lips, hands or bottom.
Sores are usually (but not always) firm, round, and painless. Because the sore is painless, you may not notice it. The sore usually lasts 3 to 6 weeks and heals regardless of whether you receive treatment. Even after the sore goes away, you must still receive treatment.
Syphilis rash
A rash can develop anywhere, but it often appears on the palms and soles. It may cause spots, blisters, or many other signs.
[5] Lesions that can be confused with the chancre of primary syphilis include herpes simplex virus infection, chancroid, fixed drug eruption, lymphogranuloma venereum, granuloma inguinale (donovanosis), traumatic ulcer, furuncle (boil), and aphthous ulcer.
Syphilitic chancres and mucous patches usually are painless, unless they become secondarily infected. Both of these lesions are highly infectious. The chancre begins as a round papule that erodes into a painless ulcer with a smooth grayish surface (see Figure 13-4). Size can range from a few millimeters to 2 to 3 cm.
A single chancre marks the onset of the primary (first) stage of syphilis, but there may be multiple sores. The chancre is usually (but not always) firm, round, and painless. It appears at the location where syphilis enters the body.
The first symptom is a painless, round, and red sore that can appear anywhere you've had sex. You can pass syphilis to others without knowing it. Washing the genitals, urinating, or douching after sex will not prevent syphilis. Syphilis is easy to treat and cure.
It is possible, but not common, to pass the sexually transmitted infections (STI) that are spread through skin-to-skin contact. Those are herpes, genital warts and syphilis. In general hand – genital contact is low risk for most STI. It is one of the safer ways you can have sexual contact.
Usually about 3 weeks after transmission a sore (called a chancre) may appear on the vagina, cervix, penis, genital area, throat or anus. The skin breaks open to reveal a red sore, (similar to a cold sore) which gets covered by a yellow or grey scab. The sores are painless and do not bleed easily.

Generally, a syphilis rash may: Appear on the palms of the hands, soles of the feet, or on the chest, arms, or back. Appear pale in color or in round, red patches. Be accompanied by small, raised bumps at the rash site.
During secondary oral syphilis, lesions that appear as white patches can develop on the inner lips and tongue. These white patches could also develop into sores or ulcers within the mouth.
The genital ulcers caused by syphilis can bleed easily, and when they come into contact with oral and rectal mucosa during sex, increase the infectiousness of and susceptibility to HIV.
During secondary syphilis, a rash may develop on one or more areas of the body. It often takes the form of rough, red or brownish spots on the soles of the feet or palms of the hands. A syphilis rash doesn't usually itch.
