Myrtle ran into the bathroom to hide from Olive Hornby because she was teasing her about her glasses. She was inside one of the cubicles when she heard someone (the basilisk) come in. She died by looking directly into the basilisks eyes in the bathroom on June 13, 1943.
How did Moaning Myrtle die? She has glasses which should've saved her, rendering her merely petrified. Rowling answered this question in an interview. She claimed that glasses don't protect a person from the basilisk's stare.
Opening the stall's door, she was about to yell at Riddle to go away, when the Basilisk stared at Myrtle and her eyes met with the monster's. Since looking into a Basilisk's eyes is a fatal act, Myrtle was killed instantly and her body fell to the bathroom floor, becoming Tom's first victim.

Myrtle's life was cut shockingly short when she was killed by the Basilisk at Tom Riddle's command. It's not surprising to me that she stayed on as a ghost.
The most likely reason why Lord Voldemort's nose disappeared and he devolved into such an evil-looking creature is that as he dabbled deeper into the Dark Arts, his appearance gradually became as distorted as what remained of his twisted soul.

According to the author, Hagrid's birthday is December 6th; events in the stories lead us to believe he is approximately 60 years old when Harry first attends Hogwarts.
