Another trick: making the eyes look bigger by injecting a baby dose of neurotoxin just underneath the eye. "If you just put one unit of Botox there," Engelman says, "it drops the lower eyelid about one or two millimeters and opens up the aperture of the eye.
Botox injected below the lower lash line can prevent the upward motion of the lower lid. The result? The illusion of larger eyes. It can also be injected into the upper eyelid to actually open the eye more, especially if there is drooping of the upper eyelids (which often makes eyes appear small).
Filler can have the opposite effect on eye bags, making them bigger and err, blue. Eye bags are tricky to treat. Even the most devout skincare aficionado can struggle to reduce their appearance using topical products alone and many find that no matter how much they sleep, their eye bags are always a concern.
It creates less distinction between the cheek and the tear trough, which causes the eye to get smaller from above, says Doris Day, MD, a board certified dermatologist in NYC. Too much filler makes the eyes appear small and squinty and, conversely, the cheeks look disproportionately big.
During the fox eye lift, the plastic surgeon lifts the outer corners of the eye and makes a very small incisions on the outside corners to stretch them slightly and give them an almond shape. The skin is then sutured into place.
"Contour eyes with a medium to medium deep shadow to the crease and outer crease of eyes," suggests the pro. "Contouring gives eyes depth and definition, making them look bigger, bolder, and perfectly shaped. Use one color for the entire crease and then go darker with a shade on top to the outer corner."
This can dramatically improve a displeasing result to improve patient satisfaction by optimizing the cosmetic result. Rarely, retracted lower eyelids can non-surgically be helped by injecting filler in the proper plane to help lift the lower eyelid upward. Dr.
In some cases, facial filler injections that are placed underneath the eyes can lead to additional swelling, and can make a festoon pocket appear more evident or larger. At times, the festoon pocket may persist for a longer period even after an unwanted facial filler product is dissolved with hyaluronidase.
"By putting fillers under the eyes, you can actually lift and shift the skin," says Dr. Marmur. "We add filler so that your skin has something to slide over smoothly instead of creating a wrinkle.
When too much product is used, swollen pockets under the eyes after fillers is a common complication. The area can appear puffy, with irregular lumps. Too much filler can also cause the Tyndall effect and blue discolouration of the skin.
Among HA fillers, Restylane was the most common (54.7%), followed by Teosyal Pure (9.8%) and Juvederm Ultra XC (9.8%). Overall patient satisfaction rate was 84.4%, with overall long-term patient satisfaction (6+ months later) of 76.7%.

Incorrect filler choice or placement can exacerbate the appearance of the cheek bags known as festoons, which can contribute to under-eye puffiness. As a general rule, permanent fillers, including Sculptra, should not be used under the eyes.
“Sometimes it can be pretty noticeable because there are a lot of vessels around the eye and the skin is thin,” she says. “That said, sometimes there is very little bruising.”
Fillers work by tissue expansion, similar to how a sponge expands when absorbing water. Fillers expand the facial tissue to add volume in the targeted areas. Lines around the eyes and hollows or dark circles under the eye can be plumped up with fillers for a more youthful appearance.
